Description
This is a request for Standard Form 330s, not a request for proposal, for Commissioning Services for the New Physical Plant that is going to be built at VA Hines Hospital. The Department of Veterans Affairs, is seeking interested firms that are classified under one or more of the following small business programs: (1) Service Disabled Veterans Owned Small Business (SDVOSB) (2) Veteran-Owned Small Business (VOSB) (3) Section 8(a), (4) HUB Zone, (5) Small Disadvantaged Business (SDB), (6) Woman-Owned Small Business (WOSB) and (7) Small Business. Scope for this work : Whole Building Commissioning services for third-party commissioning are required for the New Physical (Boiler) Plant at Hines VA Hospital. Not including in this is Building Exterior Closure commissioning that includes the roofs and walls. The successful Commissioning Agent will support through the Commissioning Process the design, construction, and post-acceptance phases of renovation, additions, or other projects. Commissioning Services shall be provided in accordance with the Department of Veterans Affairs Whole Building Commissioning Process Manual. Prospective proposers shall review the Whole Building Commissioning Process Manual available on the VA Technical Information Library at http://www.cfm.va.gov/til The selection Criteria: 1.Specialized experience and technical competence of the firm (including a joint venture or association) with the type of services required. The Commissioning Agent shall have acted as the principal commissioning agent for Physical Plant Projects supporting facilities over 100,000 Square feet. List a minimum of five (up to 10) projects of comparable type, size, and complexity which were completed within the last 5 years. 2.Specific experience and qualifications of personnel proposed for assignment to the project and record of working together as a team 3.Professional capacity of the firm in the designated geographic area of the project to perform work (including any specialized services) within the time limitations. The firm should have experience commissioning construction projects valued at $20M or higher. The evaluation will consider the availability of an adequate number of personnel in key disciplines, equipment availability, and demonstrated management procedures. 4.Past record of performance on contracts with the Department of Veterans Affairs. This factor may be used to adjust scoring for any unusual circumstances that may be considered to deter adequate performance by an A/E. 5.Geographic location and facilities of the working office(s) which would provide the professional services and familiarity with the area in which the project is located 6.Demonstrate success in prescribing the use of recovered materials and achieving waste reduction and energy efficiency in facility Commissioning. Submit two (2) copies of SF 330 with appropriate documentation by close of business March 9th, 2012.
