- This is a combined synopsis/solicitation for commercial supplies prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W81K00-12-T-M005. It is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-52 and Defense Federal Acquisition Regulation Supplement Publication notice (DPN) 20110608. The Great Plains Regional Contracting Office intends to award a Blanket Purchase Agreement(s) (BPA(s)) to the quoter(s) that are determined technically acceptable and can provide at least 50% of the items. BPA(s) under the program will have a single call limit of $25K. BPA's will be established under FAR Part 13. Supplies include endovascular devices Micrus brand name or equal. This is the only synopsis that will be provided by the government before establishing and placing calls under the BPA(s). Should the requirement result in more than one BPA holder, sole source calls will be documented and established based on physician requirements/preference. This BPA will be placed and advertised as Full and Open competition with a performance period of approximately 15 March 2012 to 14 March 2017. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112 Size standard is 500 Employees.
