SOURCES SOURGHT NOTICE REQUEST FOR INFORMATION This Request for Information (RFI) is for planning purposes only and is not a Request for Proposals (RFP), Invitation for Bid (IFB) or an obligation on the part of the Government to acquire any services. Responses to this RFI are not offers and cannot be accepted by the Government to form a binding contract. The Government reserves the right to determine how it should proceed as a result of this notice. Furthermore, those who respond to this RFI should not anticipate feedback with regard to its submission. The Government will not pay any cost incurred in response to this RFI. All costs associated with responding to this RFI will be solely at the responding party's expense. The information provided in this RFI is subject to change and is not binding on the Government. New York Harbor Healthcare System - 423 East 23rd Street, New York, NY 10010 4.Hudson Valley Healthcare System - Castle, Point, NY 12511 (Dutchess County) 5.Hudson Valley Healthcare System Route 9A Montrose, NY 10548 (Westchester County) 6.East Orange Campus VA New Jersey Healthcare System - 385 Tremont Avenue, East Orange, NJ 07018 7.Lyons Campus, VA New Jersey Healthcare System - 151 Knollcroft Road, Lyons, NJ 07939. Purpose: The CELC Implementation Center is seeking a contractor to fill an essential administrative role by providing critical management services. Scope: The vendor will be required to create and provide the following services: 1.Developing a comprehensive business plan for the Implementation Center; 2.Organizing and coordinating regular telephone meetings with Veterans Integrate Service Network (VISN) Palliative Care Program Managers, Clinical Champions, facility leaders and faculty to expand and sustain facility HPC programs through Leadership development, Team Building, Outcomes measurement and Strategic Planning; 3.Planning on-site Refresher Courses to enhance program development at selected VA facilities; 4.Supporting leadership and faculty development; 5.Coordinating Special Interest Group activities 6.Leading Quality Improvement pilots to improve results on the National VA Bereaved Family Survey.
