Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This announcement constitutes the only solicitation and is issued as a request for quote (RFQ). In accordance with FAR 5.207, all responsible sources may submit a capability statement, proposal or quotation which shall be considered by the agency. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-52 (effective 31 May 2011) and Defense Federal Acquisition Regulation Supplement (DFARS), change notice 20110623. It is the offerors responsibility to be familiar with all applicable clauses and provisions. In accordance with FAR 13.501, the Government intends to award a sole source contract to the University of Nebraska Medical Center to provide non-personal services from an accredited university to confer a Bachelor of Science and/or Masters of Physician Assistant Studies to all students successfully completing Phase I and/or Phase II of the Interservice Physician Assistant Program (IPAP), Army Medical Department Center and Schools (AMEDDC&S), Fort Sam Houston, Texas. The NAICS code for this procurement is 611310 and the size standard is $7 million. This procurement is being solicited as firm-fixed price and will be processed under FAR part 13, Simplified Acquisition Procedures in conjunction with FAR Part 12, Acquisition of Commercial Items. Requirement Summary The Army Medical Department Center and School (AMEDDC&S) requires non-severable non-personal services for an accredited university to confer a Bachelor of Science degree to all students successfully completing Phase 1 of the Interservice Physician Assistant Program (IPAP) and a Masters of Physician Assistant Studies or comparable degree, to all IPAP students successfully completing both Phase 1 and Phase 2 of the IPAP. The IPAP is a twenty-nine month program consisting of Phase 1 (four semesters (16 months) and Phase 2 (13 months)). Average enrollment is 240 students per year. The period of performance will be 1 April 2012 thru 30 September 2014 without option periods. IPAP REPRESENTATIVE (Contracting Officer's Representative). The Government will designate an IPAP representative who will: 3.1. Coordinate with the Armed Services to provide an official transcript for all colleges/universities attended by each student, for the contractor's review and GPA calculation prior to the military selection board convening date. 3.1.2. Provide all instruction, texts, facilities, materials, instructors, and local administrative support for IPAP students. 3.1.3. Provide student grades to the contractor, for posting to official transcripts. 3.1.4. Provide space and personnel to assist the contractor during on-site registration at IPAP. 3.1.5. Forward staff Curriculum Vitae for faculty appointments to contractor for review not later than 30 calendar days prior to assignment to IPAP. 3.1.6. Forward curriculum and educational objectives to the contractor for review within 30 days of quarterly academic committee meeting. 3.1.7. Provide timely notifications of changes in student status to the contractor; i.e., recycles and disenrollments within 14 days of the of the recycle/disenrollment process. 4. THE CONTRACTOR SHALL: 4.1. Provide all necessary administrative services for the IPAP students to receive college credits and a baccalaureate degree and a Master's degree. 4.2. Be accredited by an appropriate regional accrediting body and be authorized to confer the degree of Bachelor of Science and Master of Physician Assistant Studies (MPAS) (or equivalent Masters Degree). 4.3. Have a Physician Assistant Program as part of its university system that is approved by the Accreditation Review Commission for Education of the Physician Assistant (ARC-PA), and is a member of the Association of Physician Assistant Programs (APAP/now Physician Assistant Education Association [PAEA]), and has trained Physician Assistant students for a minimum of five (5) years. 4.4. Be informed and have the opportunity to review and provide input/feedback regarding any major curriculum changes made to the IPAP. 4.5. Be permitted to review field evaluation reports and annual course reviews upon request. 4.6. Assist any IPAP student to complete his/her degree requirements through transfer into the contractor's PA program, if the student was disenrolled in IPAP for medical reasons after completion of Phase 1. All such students must provide evidence that he/she can reasonably be expected to complete the equivalent of IPAP Phase 2 training in a civilian environment with the contractor having unilateral decision-making authority. 4.6.1. After initial enrollment, shall not invoice on an individual student basis for any administrative costs to include matriculation or enrollment fees. 4.6.2. Not invoice individual students for any services provided under the contract. 4.7. Review student transcripts prior to military selection board and calculate overall Grade Point Average (GPA) and Science GPA and report these along with a determination of the candidate's eligibility for admission to the college/university to the military selection boards, at the request of individual Armed Services. 4.8. Review student transcripts prior to registration to ensure that necessary prerequisites are fulfilled, at the request of individual Armed Service. 4.9. Accept the admission of students from any uniformed service to the IPAP located at the AMEDDC&S, Fort Sam Houston, Texas, that have at least 60 hours academic credit from an accredited college/university, 30 hours of which must be in residence. The remaining hours may be derived from College Level Examination Program (CLEP) correspondence courses, Defense Activity for Nontraditional Education Support (DANTES), military training credit on Army American Council on Education Registry Transcript System (AARTS) transcript, Sailor/Marine American Council on Education Registry Transcript (SMART), et The acceptability of prerequisite courses shall be determined by agreement between the contractor and the IPAP representative. The contractor shall waive any requirements for courses of a state or local nature for IPAP students. 4.10. Provide personnel for on-site (one person for one day at IPAP location) registration and orientation briefing for new students three times per year during the orientation week of each new class. 4.11. Admit all registered IPAP students as students in full standing with all rights and privileges afforded to all contractor's students and provide a copy of transcripts to include GPA calculations. 4.12. Post grades in student transcripts and maintain transcripts in the same manner as for all contractors' students. An official degree-posted transcript will be sent to IPAP representative upon graduation. 4.13. Confer a Bachelor of Science degree to all IPAP students successfully completing Phase 1 of the Interservice Physician Assistant Program and a Master of Physician Assistant Studies or comparable degree, to all IPAP students successfully completing both Phase 1 and Phase 2 of the IPAP. Actual degrees/diplomas shall be mailed to Registrar, U.S. AMEDDC&S, Fort Sam Houston, Texas, no later than three weeks prior to graduation. 4.14. Accept the grading system used by IPAP, as follows: A+ = 97-100 A = 90-96 B+ = 87-89 B = 80-86 C+ = 78-79 C = 75-77 F = <75 4.15. Provide annual on-site visit to IPAP at Fort Sam Houston, TX by an appropriate faculty member or members (for one to two days) to observe/review curriculum, teaching methods, and course content of the IPAP program. A written report of observations shall be forwarded to the AMEDD C&S, IPAP representative to be included in the ARC-PA self study process, if deemed necessary. Participate in the IPAP reaccreditation site visit, if deemed necessary. 4.16. Participate in the IPAP annual curriculum review to ensure course materials, curriculum hours and credit recommendations will meet college/university requirements. 4.17. Offer Adjunct faculty status to Phase 1 instructors and Phase 2 Medical Directors and Clinical Coordinators of the IPAP. 4.18. Provide information regarding university programs via a website address or a hardcopy catalog, on request. 4.19. Publish IPAP course description via a website address or a hardcopy catalog. 4.20. Record changes in student's status, i.e.; recycled into later class or disenrolled when notified by IPAP/AMEDD C&S representative. 4.21. Provide faculty development workshop (by appropriate faculty member or members for one or two days) at least annually through on site visits or video teleconferences. 4.22. Review and grade the final research paper required of Master's candidates provided such papers are submitted to the contractor at least 120 days prior to the student's expected date of completion of IPAP Phase 2. All graded papers shall be returned to the students with comments from the professor prior to graduation. 4.23. CONTRACTOR MANPOWER REPORTING (CMR): Contractor Manpower Reporting (CMR). ACCOUNTING FOR CONTRACT SERVICES - The Office of the Assistant Secretary of the Army (Manpower and Reserve Affairs) operates and maintains a secure Army data collection site where the contractor will report ALL contractor manpower (including subcontractor manpower) required for performance of this contract. The contractor is required to completely fill in all the information in the format using the following web address https://cmra.army.mil/. The required information includes: (1) Contracting Office, Contracting Officer, Contracting Officer's Technical Representative (COTR); (2) Contract number, including task and delivery order number; (3) Beginning and ending dates covered by reporting period; (4) Contractor name, address, phone number, e-mail address, identity of contractor employee entering data; (5) Estimated direct labor hours (including sub-contractor); (6) Estimated direct labor dollars paid this reporting period (including sub-contractor); (7) Total payments (including sub-contractor); (8) Predominant Federal Service Code (FSC) reflecting services provided by contractor (and separate predominant FSC for each sub-contractor if different); (9) Organizational title associated with the Unit Identification Code (UIC) for the Army Requiring Activity (the Army Requiring Activity is responsible for providing the contractor with its UIC for the purposes of reporting this information); (10) Locations where contractor and sub-contractors perform the work (specified by zip code in the United States and nearest City, Country, when in an overseas location, using standardized nomenclature provided on website); (12) Presence of deployment or contingency contract language; and (13) Number of contractor and sub-contractor employees deployed in theater this reporting period (by country). As part of its submission, the contractor will also provide the estimated total cost (if any) incurred to comply with this reporting requirement. Reporting period will be the period of performance not to exceed 12 months ending September 30 of each government fiscal year and must be reported by 31 October of each calendar year. IPAP Program Document 1. FOREWORD: 1.1. The Interservice Physician Assistant Program (IPAP) curriculum and requirements meet the accreditation standards of the Accreditation and Review Commission for Education of the Physician Assistant (ARC-PA). Satisfactory completion of Phase 1 and 2 indicates the student has met the basic requirements to be a competent physician assistant in accordance with nationally recognized educational standards. Additionally, students must meet the physical and regulatory requirements to become an officer IAW each service's policies and regulations. 1.2. It should not be construed that the graduating students possess all the skills and knowledge to function in a specialty area. Final qualification of the individual, with respect to job proficiency, rests with the individual's local credentialing authority. 2. COURSE DATA 2.1. COURSE MISSION: To provide Physician Assistant students with skills and knowledge of primary health care delivery necessary to assume the duties and responsibilities of a certified physician assistant. 2.2. COURSE SCOPE: 2.2.1. Phase 1. Didactic instruction that includes the following: Anatomy, physiology, biochemistry, laboratory sciences, microbiology, pathology, patient evaluation, pharmacology, psychology, infectious diseases, neurology, dental sciences, cardiology/EKG, pulmonology, gastroenterology, endocrinology, ophthalmology, otorhinolaryngology, rheumatology, Advanced Cardiac Life Support, orthopedics, obstetrics, gynecology, dermatology, nephrology, pediatrics, geriatrics, surgery, emergency medicine, radiology, military public health, PA professional issues, biomedical ethics/medical law, advanced trauma management, and research methodology. 2.2.2. Phase 2. Clinical clerkship with rotations in the following areas: surgery, dermatology, obstetrics, gynecology, orthopedics, psychiatry, internal medicine, otolaryngology, allergy, pediatrics, ophthalmology, emergency medicine, outpatient/family practice, and electives. 2.3. SECURITY CLASSIFICATION: Unclassified 2.4. COURSE LENGTH: Phase 1 - 16 months; Phase 2 - 13 months. 2.5. LOCATION OF INSTRUCTION: Phase 1 - Academy of Health Sciences, Fort Sam Houston, TX; Phase 2 - Select uniformed medical treatment facilities/service hospitals in the United States. 2.6. STUDENT CAPACITY: Class size: 80; Convening Rate: 3 concurrent classes per year. 2.7. PHASE 1 AND PHASE 2 CURRICULUM APPROVAL AUTHORITY: Interservice Training Advisory Board representatives for IPAP, Fort Sam Houston, TX 2.8. PRIMARY MODE OF INSTRUCTION: Phase 1 - Group paced; Phase 2 - Clinical 2.9. ACCREDITATION: The IPAP must maintain accreditation with the Accreditation and Review Commission for Education of Physician Assistants (ARC-PA) and affiliation membership with the Association of Physician Assistant Programs (APAP). 3. STUDENT DATA SHEET: 3.1. PERSONNEL PHYSICAL REQUIREMENTS/QUALIFICATIONS: Army officer appointments are established in Chapter 2, AR 40-501, including height and weight standards IAW AR 600-9. Navy personnel must be qualified for transfer IAW article 15-57 of the Manual of the Medical Department, Chapter 7.01 of the Enlisted Transfer Manual and Chapter 2 of the Officer Transfer Manual. Navy members requiring medical attention, including pregnancy, shall not be transferred to this school. Air Force personnel must meet physical commissioning listed in AFI 36-2005. 3.2. SECURITY CLEARANCE: None 3.3. EDUCATIONAL REQUIREMENTS: Minimum of 60 transferable college credits from accredited colleges/universities, of which at least 30 must be in-residence hours and the remainder may be credit for College Level Examination Program (CLEP) subject examinations and military experience, Defense Activity for Nontraditional Education Support (DANTES) Correspondence Courses, ACI-Proficiency Examination Program. Mandatory courses include 6 hours of English, of which 3 must be composition; 3 hours of college algebra or higher level mathematics; 3 hours of human anatomy (no lab required) and 3 hours of human physiology (no lab required) or 6 hours of combined anatomy and physiology; 3 hours of psychology; 6 hours of general chemistry or higher level chemistry (no lab required); and 6 hours of humanities/social sciences. Must have a sound working knowledge of written and spoken English. 3.4. OBLIGATED SERVICE: Active Army, 48 months from date of course completion (Phase 1 and 2). Army Reserve, 6 years from date of course completion (Phase 1 and 2). Service time remaining for Army National Guard is governed by NGR 350-1. Air Force and Navy personnel incur a 48-month obligation from date of course completion (Phase 1 and 2). 3.5. OFFICER DESIGNATOR EARNED: Army AOC 65D; Navy 2305; Air Force 42G1. 3.6. RELATED AND/OR FOLLOW-ON TRAINING: Recertification requirements established by the National Commission on Certification of Physician Assistants (NCCPA), including 100 hours of approved Continuing Medical Education every 2 years, and successful completion of the Physician Assistant National Recertification Examination (PANRE) or Pathway 2 examination every 6 years. 3.7. SPECIAL INFORMATION: Graduates will be required to successfully complete the NCCPA entry-level examination, Physician Assistant National Certification Examination (PANCE), and maintain NCCPA certification in accordance with service specific regulations.
