Description

This is only a request for information, do not submit pricing. The Government is only looking for sources that are available to meet the requirements described below. Vendors that can meet the requirements described below are to submit company information: Vendor name Address Email and phone number Cage Code & DUNS Number Business size Submit all information electronically to corey.labbe@va.gov Phone inquiries will not be accepted. All responses must be received no later than 1600 on February 14, 2012. Carmeda coated bio-active Sterile Custom Surgical Heart Packs are needed for the Department of Veterans Affairs, NF/SG VA Health Care System, 1601 SW Archer Road, Gainesville, FL 32608. The requirement is estimated for a Base period of March 01, 2012 thru February 29, 2012 plus 4 option years. Quantities are estimated at 180 cases to be ordered "as needed basis". The custom packs must include various lengths of ¼, 3/8, and ½ in tubing, tubing end caps, one way valves, clamps, and stopcocks; various tubing connectors; prebypass and arterial filters; cardiotmy reservoir; hempconcentrator. The total pack will also contain one oxygenator with hard-shell venous reservoir.
