- UNICOR intends to enter into an Indefinite Delivery Indefinite Quantity (IDIQ) contract with one (1) base year and four (4) option years if exercised type contract for PLM0008-1/4SBP"X49"X97",FP612K,MOROCCO-DBL-FACED SBP = SOLID BLACK PHENOLIC for FCI Ashland KY. Solicitation number is FN1350-12 and this solicitation is issued as a Request For Proposal.. The minimum delivery order quantity is 100 sheets and the maximum delivery order quantity is 600 sheets. Delivery lead time is 21 days. This is a small business set aside requirement. Therefore, only offers received from small businesses will be considered for award. NAICS code associated with this requirement is 326130. The small business size standard for NAICS code 326130 is 500 employees. Offerors are advised that the Government intends to evaluate offers and award on a SINGLE AWARD BASIS. Offerors shall submit signed and dated offers to UNICOR, Federal Correctional Institution, PO Box 1500, White Deer, PA 17887, Attn: Tiffani Balestrini if submitting via US Postal Service or UNICOR, Route 15 2 Miles N. of Allenwood, Allenwood, PA, 17810 Attn: Tiffani Balestrini if submitting via FEDEX or UPS. Specific item specifications and quantities will be listed in solicitation. All offerors must complete the Business Management questionnaire in Section L of solicitation document with three references. References must be for same or similar items. All Offerors must complete the ACH form and the CCR Registration, which can be found on UNICOR's webpage at www.unicor.gov. For the ACH form click on business commercial opportunities, navigate to, procurement forms and click on ACH form and print. For CCR Registration scroll down the page to Central Contractor Registration and complete internet based form. Offerors may also submit proposals via fax at 352-330-8152 or email to tbalestrini@central.unicor.gov. Email proposal shall be sent as an Adobe PDF files and offerors shall contact Tiffani Balestrini to verify receipt of proposal. All questions shall be asked in writing and shall be submitted to Tiffani Balestrini at tbalestrini@central.unicor.gov and Tatea Cavanaugh, tcavanaugh@central.unicor.gov the email subject line shall state the Solicitation number FN1350-12. For email and fax proposals, an originally signed SF1449 must be received by the contracting officer within 1 day of offer closing date and time. Offerors shall submit 2 copies of the written proposal. All offerors must acknowledge all amendments if any. Requirements for submission of samples will be listed in detail in the full solicitation. Offers received after the exact time and date specified will not be considered. The Government intends to award a contract resulting from this solicitation to the responsible offeror's whose offer represents the best value to the government. The following factors shall be used to evaluate proposals: List in order of importance: Adherence to specifications (full listing of this evaluation criteria will be listed in solicitation), past performance and price. A written notice of award or acceptance of an offer will be mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. No copies of the solicitation will be mailed. All responsible offerors may submit a proposal which will be considered.
