Description

The Department of Veterans Affairs, VISN 7 Network Contracting Activity, Charleston VA Medical Center currently intends to solicit, negotiate and award a sole source contract to Cook Medical Inc.; Bloomington, IN for the purchase of supplies for interventional radiology procedures located at the Ralph H. Johnson Veteran Affairs Medical Center (VAMC), 109 Bee St., Charleston, SC 29401, under the authority of FAR 6.302-2. This notice of intent is NOT A REQUEST FOR COMPTETITIVE PROPOSALS. No solicitation documents are available and telephone requests will not be honored. However, any interested parties shall respond to the notice within five (5) days after date of this publication to be considered by the Government. Responses received will be evaluated; however, any determination by the Government not to open the requirement to competition is based upon responses to this notice and is solely with the discretion of the Government. If no responses are received, the government will proceed with the sole source negotiations with Cook Medical Inc. Any responses shall be emailed to Benford Brundage at Benford.Brundage@va.gov. Interest parties may identify their experience in providing this equipment by submitting the following information. (1) STATEMENT OF CAPABILITY: submit a brief description [three (3) pages or less] that shows your company can provide the required items/equipment. Include past experience in providing the items/equipment to the VA, other Government (Federal or State) agency, or for a private medical facility. Please specify your ability to deliver as stated. (2) BUSINESS SIZE AND SOCIO-ECONOMIC STATUS relative to NAISC 339112, Surgical and Medical Instrument Manufacturing (size standard of 500 employees). : (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if you firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub-zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate If your firm is a certified Service-disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your firm. (h) State whether your firm is registered with the Central Contactor Registration (CCR) at http://www.ccr.com and/or the VetBiz Registry at http://vip.vetbiz.gov/. If not, please NOTE: any future solicitation could only be awarded to a contractor who is registered in CCR and to receive award based on VOSB or SDVOSB status you must be registered in the VetBiz Registry. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government. All responses must be received not later than noon CST on 2/6/2012.
