Description
STATEMENT OF WORK 1.SCOPE The contractor shall provide all personnel, equipment, tools, materials, supervision, facilities, transportation, and other items and services necessary to perform refilling of government owned Medical Gas Cylinders as defined in this Statement of Work (SOW) for North Florida/South Georgia Veterans Healthcare System. All cylinders are stamped and identified as government owned. Vendor will annotate delivery packing slip with type, size, and quantity of cylinders picked up and returned (filled). On returned government-owned cylinders, the contractor will provide a certificate of purity from the supplier for any medicinal gases. If cylinder requires hydrostatic testing or repainting, the contractor must get approval from North Florida/South Georgia Health System identifying charges prior to service, if feasible. If the cylinder fails hydrostatic testing or any other damage has rendered the cylinder unserviceable, the contractor must identify and return tagged cylinder as such to the SPD section. Compliance with any Certification of transporting cylinders of any type arising from local, state or Federal Department of Health Regulations is the responsibility of the contractor; and may be requested as proof to the requiring activity at any time. 2.PERFORMANCE PERIOD This is a recurring requirement. The government anticipates the award of a five-year agreement resulting from this solicitation, consisting of a base period and four one-year option periods. Option periods may or may not be exercised; they are at the discretion of the government. AUGUSTINE (CLIN004) 1 NITROGEN LIQUID (240 LITTRE TAKE) OCALA CBOC (CLIN005) 1 NITROGEN LIQUID (240 LITTRE TAKE) 6.SPECIAL SECURITY REQUIREMENTS A.VA Police will issue photo identification badges directly to contractors' and sub-contractors' superintendents and foremen. These ID badges must be worn by these individuals at all times while on VA property. B.Contractors' supervisory personnel will be responsible for acquiring temporary ID badges from VA's Engineering Service, for all other staff, employees, and visitors associated with this project. These temporary ID badges shall be managed in accordance with procedures described in Engineering's Badge Control Agreement (attached), and must be worn by Contractor's non-supervisory staff, employees, and visitors at all times while on VA property. C.Engineering's Badge Control Agreement will be initiated at the start of the project, when Contractors' supervisory personnel acquire temporary ID badges for the first time. Final payment and project close-out shall be contingent upon satisfactory execution of this Badge Control Agreement. 7.QUALIFICATIONS/CERTIFICATION A.The contractor shall be qualified and have the required levels of professional and technical experience to perform the work required by this statement of work. B.Compliance with any Certification of transporting cylinders of any type arising from local, state or Federal Department of Health Regulations is the responsibility of the contractor; and may be requested as proof to the requiring activity at any time. 8.SCHEDULING AND HOURS OF WORK A.Hours of work. The contractor will perform work during normal working hours unless specifically authorized by the Contracting Officer's Technical Representative (COTR), or otherwise specified in the contract. Normal duty hours are 8:00 a.m. to 4:30 p.m., Monday through Friday, except U.S. Federal Holidays, which include New Years Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas or any other day specifically declared by the President of the United States to be a national holiday. B.Upon the customer's request, and mutual agreement with the contractor, pick-up/deliveries may be required outside of normal hours at premium charges, if applicable. If weather conditions exist which prevents contractor's delivery within the specified hours or operations, the VA Medical Center shall be notified within one hour of the estimated time of arrival. An alternate method of pickup and delivery of medical cylinders shall be provided by the contractor should this situation arise if feasible. Additionally, the Government, as proof of performed service, shall retain a copy of the pick-up/delivery ticket. 9.INSPECTIONS All work shall be inspected by the VA technical representative.
