Description
SCOPE OF SERVICES: The contractor shall provide easy to use preprinted Scranton forms for Environment of Care Round (EOC) teams to use in conducting rounds or audits, forms MUST be customizable to VAMC, Miami, Florida. The vendor shall provide professionally graphed documents resulting from rounds or audits which identify trends. The vendor shall work with VAMC-Miami to gather and structure all relevant information required to implement readiness rounds; this includes but is not limited to, developing annual schedules and inspection questions that will be incorporated into the process. Vendor shall provide monthly scheduled forms to VAMC, with a complete packet providing a comprehensive look at the facilities EOC shortcoming and deficiencies. The vendor must manage all items that have been identified deficient and track them until they are corrected and closed out. The service should also include: 1) Web-based reporting with unlimited access from any PC with internet access to our account, 2) Run different reports and graphs with 1 click and no work involved to trend and analysis the results, 3) Meet requirement for VAMC Miami, Florida reporting, Joint Commission and OIG/SOARS performance improvement plans and trending, 4) Identification of how many deficiencies and include information such as; items corrected, # days outstanding, subject area of deficiency and management officials responsible for correction, 5) Electronically generated notifications of deficiencies to the responsible official for corrective action; prefer real time/instant data reporting system or a turn-around time of no more that one day from readiness rounds completion due to limited time frame for close out of deficiencies. Forms are a part of the reporting system then time frames for returning the initial rounds forms and the corrective action forms outlining the corrective action taken should be followed up in the manner stated below. Initial rounds forms should be followed up if not received by the next business day. If the rounds forms have not been sent by day 2 then the next higher level should be contacted electronically. For corrective action taken, form return via electronically should be implemented on the day established by the medical center. On the due date if the responsible official has not returned his/her form, a follow-up message should be sent electronically. If by the next day there is a delay in responding, then the supervisor will be contacted electronically and if no response is received by the next day then an electronic message should be sent to the next higher level management official. Vendor must demonstrate knowledge, skills and abilities of a Continuous Readiness Program in a Healthcare environment. Vendor must possess the knowledge, skills and ability to train designated medical center and coordinate the first review to ensure the program is used correctly.
