Description
This is NOT a solicitation announcement. This is a Sources Sought Notice only. The purpose of this sources sought is to gain knowledge of potential qualified sources and their size classifications (Service Disable Veteran -Owned, Veteran - Owned Small Business, Women-Owned Small Business, HubZone 8(a), 8(a) HubZone, Small Business, Emerging Small Business, Small Disadvantage Business (SDB), and Large Business) relative to the North American Industry Classification Code (NAICS) 621111. The Small Business Size Standard is $10.0 M. Responses to this sources sought notice shall not constitute responses to the solicitation, which is not currently available. As stipulated in FAR 15.201, responses to this notice are not considered offers and cannot be accepted by the Government to form a binding contract. NO solicitation exists; therefore, do not request a copy of the solicitation. The decision to solicit for a contract shall be solely the Government's discretion. This sources sought notice is not to be construed as a commitment by the Government, nor will the Government pay for the information submitted in response. Respondents will not be notified of the results of any government assessments. The information received will be utilized to assist in formulating the strategy for competitive procedures. The type of solicitation issued and the manner of advertisement will depend on the responses to this sources sought. Description The Department of Veteran Affairs, Robley Rex VA Medical Center (VAMC), Louisville, KY, intends to award an Indefinite Delivery Indefinite Quantity Firm Fixed Price contract for Radiation/Oncology Therapy services. Personnel assigned by the Contractor to perform services covered by this contract shall be Board Certified in Radiation Oncology and a Board Certified Radiation Physicist. Contractor shall provide outpatient professional and technical radiation therapy services at their facility for Veterans as well as providing the materials, supplies, equipment and qualified supervision. Comprehensive radiation therapy would include, but not be limited to, the initial evaluation, treatment plan, simulation, medical radiation physics, dosimetry, treatment devices, special services, clinical treatment management, and a review of pertinent x-rays and laboratory results as well as the patient's medical record to include follow-up care associated with the course of treatment. The treatment plan shall include the type of radiation to be administered, the site, the dosages, frequency, the treatment dates and the follow-up examination date(s). The contract period will be a base period (Date of award - June 30, 2013) and four (4) one (1) year option periods. Must reflect Data Universal Numbering System (DUNS) commercial and Government Entity (CAGE), and Small Business Administration Certification of Small Business Type. " Name of firm with address, phone and point of contact. " Statement of Capability (SOC) stating your skills, experience, knowledge, and equipment required to perform the specified type of work. This should include documentation of past specialized experience and technical competence in fabricating similar products, and up to three (3) similar past projects for which your firm was the prime contractor.
