- Solicitation package for the replacement of existing Radiographic and Digital Fluoroscopic base system at the Claremore Indian Hospital This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number CLA-12-R-0009 is for a Contractor to 1) purchase or lease of a new system to replace the existing GE XC Precision 500 W/32 Aurora (SRF118), System ID 918CI1500D2 system, 2) coordinate the de-installation of the current system; 3) installation of a new radiographic and Digital fluoroscopic base system; 4) training staff at the Claremore Indian Hospital in the operation of the successful offeror's system and 5) preventive maintenance/maintenance. The responses will propose three separate offers at 1) purchase and PM/Maintenance for four option years; 2) Lease to purchase and PM/Maintenance for four option years and 3) Lease and PM/Maintenance for four option years. The attached solicitation is issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-55. This acquisition is full and open competition. The NAIC code is 423450 and the small business size standard is 100 employees. Offerors are to include a completed copy of the provision found at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. The clause found at FAR 52.212-4, Contract Terms and Conditions - Commercial Items applies to this acquisition. The clause found at FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items applies to this acquisition. FAR 52.212-1 Instructions to Offerors - Commercial applies to this acquisition. Offers are due February 24, 2012, 4:30 p.m. CST at Claremore Indian Hospital, Support Services, 101 South Moore Avenue, Claremore, Oklahoma 74017.
