- THIS ANNOUNCEMENTCONSTITUTES THE ONLY SOLICITATION: A SEPARATE WRITTEN SOLICIATION WILL NOT BE ISSUED. QUOTES ARE BEING REQUESTED. The Centers for Medicare and Medicaid Services intends to award using the Simplified Acquisition Procedures (dollar threshold of $150,000), a fixed priced purchase order at a fixed priced to purchase Crucial Conversations Participant Tool Kit on a sole source basis to Vital Smarts, 282 West River Bend Lane, Suite 100, Provo, UT 84604-5799. The period of performance is 1 year from date of award. The North American Industry Classification Code (NAICS) is 611430. The Request for Quote (RFQ) is being set-aside for small business concerns. Responses must be sent electronically to Lucille.Lee@cms.hhs.gov by February 8, 2012 @ 11:00am. Please submit pricing for one base year and two option years for: 200 - Crucial Conversations Participant Toolkit materials, including: 224 page training workbook, Crucial Conversations Action Planner, Contract and model cards, a copy of Crucial Conversations: Tools for Talking when Stakes are High, Crucial Conversations audio companion (6-ed set for strengthening skills), Certificate of Completion, Master Mission on-line training tool, Subscription to the Crucial Skills Newsletter, a weekly email service and Follow-up materials that include a self-scoring under stress self-assessment, video examples, and other downloads. All responses/quotes submitted shall remain valid for a period of 180 days. Please submit your quotes in whole dollar amounts only.
