- This Notice is being issued to provide notice that the Department of Veterans Affairs, intends to solicit and award a firm-fixed price contract for PharmE Waste Wizard Services, NAICS code 562112 and the small business size standard is $12.5 million. This procurement is set-aside 100 percent for Service Disabled Veteran Owned Small Business (SDVOSB). All interested Service Disabled Veteran Owned Small Business (SDVOSB) firms must be registered in the Central Contractor Registration (CCR) at www.ccr.gov/ and registered at the VetBiz Vendor Information Pages: www.vip.vetbiz.gov/Public/Register/Benefits as a SDVOSBfirm. The Contractor shall provide all equipment, materials, and labor needed to conduct an inventory of all formulary pharmaceuticals and unused pharmaceuticals administered at Michael E. DeBakey VA Medical Center, 2002 Holcombe Blvd., Houston, Texas 77030 (MEDVAMC) pharmaceutical dispensing operations/facilities, including approximately nine CBOCs and one Domiciliary to qualify the amount of medications the facility is disposing of via a fast and user-friendly computerized online inventory tracking control system that is available to pharmacy staff and other key hospital personnel. As a minimum, the inventory database shall identify include all the following: a.National drug code (NDC); b.Brand name; c.Generic name; d.Manufacturer; e.Dosage(s); f.Form (i.e., liquid, tablet, cream, etc) g.Expiration date (Different forms of the same medication may expire more quickly or remain in stock longer; h.Package size; i.Recommendations to reduce unused pharmaceuticals; j.Recommendations to properly manage unused and used pharmaceuticals; k.Applicable Federal, State, or local requirement concerning the proper management of unused and used pharmaceuticals; l.Recommendations on proper segregation, storage and disposal of all pharmaceuticals used in the MEDVAMC facilities (Hazardous, Non-hazardous, P, U, & D-listed waste); PERIOD OF PERFORMANCE: The anticipated period of performance for this procurement will be a eight (8) months base period of February 15, 2012 through September 30, 2012, plus four (4) additional one-year option periods that may be exercised at the discretion of the Government. The solicitation for this service is anticipated to be issued on or around 1/31/2012.
