- Difficult to Place Patients in Nursing Home Care The U.S. Department of Veterans Affairs Minneapolis VA Health Care System (MVAHCS) is seeking information through this Request for Information (RFI) no. VA263-12-I-0180 regarding the availability of nursing home care to serve specific populations of patients. Veterans served will include those that need secured environments, such as locked units, dementia care units. Veterans referred will include those with dementia, mental health diagnoses, and behavioral issues. Placement will be sought after acute care hospitalization at the MVAHCS. Facilities should be able to accept admission of the patients within 24 hours of request from MVAHCS. This is a Request for Information (RFI), only. This is not a solicitation to buy, but an exploratory RFI for purposes of the MVAHCS' market research. A separate Request for Proposals (RFP) will likely follow. Responses are due Thursday, February 17, 2012, 3:00 p.m., CST. What to submit to the MVAHCS. Please respond with a descriptive cover letter indicating whether your organization meets the standards below and/or what standards your organization does not meet, if there are any. Also include a capabilities statement about your organization and a contact person with his/her contact information, including an e-mail address. The qualifying standards. The MVAHCS is conducting this Request for Information (RFI) so that we may collect information from nursing home vendors that meet the following criteria/standards for quality of care: 1.Medicare Certification 2.Full and unrestricted license for skilled nursing facility care 3.Meet state requirements for medical liability insurance 4.Exclusionary criteria are as follows: a.Three (3) level "G" or worse deficiencies in the current survey. b.The total number of health requirement deficiencies are twice the State average in the current survey. c. A level "E" or higher deficiency in the current survey, in one of the following areas: Restraints (Federal Tag, F221 or F222); Abuse - F223; Staff Treatment of Patients (includes background check) - F225 or F226; Dignity - F241; or, Licensure - F491. d.RN Hours per resident day are below the state average. e.Total Nursing Staff hours per resident day are below the state average. f.A level "E" or higher deficiency in the current survey in one of the following areas: Nursing Services - F353; Nursing Aide Training - F494 or F495 or F496; Regular In-Service Training - F497; Proficiency of Nursing Aides - F498; or Staff Qualifications - F499. g.Six (6) or more of the CMS Quality Measures listed in Nursing Home Compare fall above the state average.
