- Attachment 2 - Statement of Work (SOW) Attachment 1 - Pricing Schedule and Incorporated Clauses This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. The solicitation number for this procurement is FA4830-12-Q-0001 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-55, Defense Acquisition Circular (DAC) 91-13 (Correction) and Air Force Acquisition Circular (AFAC) 2012-0104. The North American Industry Classification System (NAICS) is 813110. The small business size standard is $7.0 million. This acquisition will be set aside 100% for small businesses in accordance with Federal Acquisition Regulation (FAR) 19.502-2(a). Description: Work to be performs includes the contractor to provide all personnel and supervision necessary to perform duties required for the position of Protestant Parish Coordinator (PPC) as defined in the Statement of Work (SOW) attached. The Contractor shall perform to the minimum standards of performance in this contract. Additionally, the PPC shall be an active participant in the Moody Air Force Base Protestant Parish and shall have a strong knowledge and a theological grasp of the Protestant faith; a working knowledge of the Air Force Chaplaincy and the military environment is preferred. This position requires a minimum of twenty (20) hours per week at the chapel offices, and includes Sunday and other days of the week in observances in support of the Protestant Parish. See Attachment 2, SOW for full description of the requirement. Quotations must be submitted on the attached Pricing Schedule Provision at FAR 52.212-1, Instruction to Quoters Commercial Items applies to this acquisition. Request for Quotation (RFQ) should be submitted and shall contain the following information: RFQ NUMBER; TIME SPECIFIED FOR RECEIPT OF QUOTES; NAME, ADDRESS; TELEPHONE NUMBER OF QUOTER; TERMS OF THE EXPRESSED WARRANTY; ANY DISCOUNT TERMS AND ACKNOWLEDGEMENT OF ALL SOLICITATION AMENDMENTS (if applicable). Contractors shall insert unit prices in the attached pricing schedule and sign below indicating prices to be used. By signing, the quoter agrees with all terms, conditions, and provisions included in the solicitation. Quotes shall also contain all other documentation specified herein. PERIOD OF ACCEPTANCE OF QUOTE: The quoter agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of quotes, unless another time period is specified in an addendum to the solicitation. LATE QUOTES: Request for Quotations or modification of quotes received at the address specified for the receipt of quotes after the exact time specified for receipt of quotes will not be considered. EVALUATION/AWARD: IAW FAR 52.212-2, Evaluation-Commercial Items, the Government will award a purchase order to the quoter whose price is judged to represent the best value to the Government. QUOTER MUST BE CENTRAL CONTRACTOR REGISTERED (CCR). Information concerning CCR requirements may be viewed via the internet at http://www.ccr.gov or by calling the CCR Registration Centers at 1-888-227-2423. All quoters must be Central Contractor Registered (CCR) and online representation and certification application (ORCA) must be activated. ORCA registration for on line representation and certification requirements may be viewed via the internet at www.bpn.gov. QUOTERS MAY INCLUDE A COMPLETE COPY OF THE PROVISION AT FAR 52.212-3 QUOTER REPRESENTATIONS AND CERTIFICATIONS - COMMERCIAL ITEMS WITH THEIR OFFER, which can be downloaded from the internet or requested in writing via email to the Point of Contact (POC). Quoters that fail to furnish the required representation information via submission or ORCA registration, or reject the terms and conditions of the solicitation, may be excluded from consideration.
