SOURCES SOUGHT NOTICE - CARDIOLOGY SERVICES THIS ANNOUNCEMENT IS NOT A REQUEST FOR PROPOSAL Place of Performance: James E. Van Zandt Veterans Affairs Medical Center 2907 Pleasant Valley Boulevard Altoona, PA 16602-4377 The James E. Van Zandt VA Medical Center is seeking qualified firms who may be interested in performing the attached medical services as identified in the Statement of Work excerpt listed below: The Contractor will provide the services of.32 FTEE Cardiologist to perform the services required within the Performance Work Statement at the James E. Van Zandt VA Medical Center (Altoona VAMC), Altoona, PA. The Cardiologist shall provide regularly scheduled outpatient clinic services between the hours of 8:00 A.M. and 4:30 P.M., Monday through Friday, excluding Federal Holidays for approximately three hours per week. Specific outpatient clinic day(s) and hours will be determined after award. No outpatient clinic services will be required outside of the established day(s) and hours. The Cardiologist shall perform all stress tests and interpret all cardiology tests performed in the cardiology lab between the hours of 8:00 A.M. and 4:30 P.M., Monday through Friday, excluding Federal Holidays for approximately 10 hours per week. Specific cardiology lab day(s) and hours will be determined after award. The cardiologist shall interpret EKG's as requested by the medical center on an as needed basis. The Cardiologist shall perform inpatient consultations and follow-up visits within the requested timeframe The Cardiologist shall perform inpatient consultations and follow-up visits according to the urgency level indicated in the consult request (i.e. STAT is within 2 hours, Today, within 24 hours, within 48 hours, within 72 hours). The contractor shall provide on-call services 24/7, 365 days a year. All services shall be performed at the Altoona VAMC at 2907 Pleasant Valley Blvd; Altoona, Pennsylvania. The Cardiologist will be granted clinical privileges for the Altoona VAMC based on his/her specific qualifications and credentials. Work will commence immediately upon completion of credentialing and privileging and one week orientation program. The Cardiologist will support direct patient care needs at the Altoona VAMC. The Cardiologist will be required to work cooperatively with other physicians, nurse practitioners, physician assistants, registered nurses, LPNs, and a variety of allied health professionals to deliver comprehensive health care services to adult patients. 1.Definitions: The terms listed below have the following meanings. a.Contractor - in this document Contractor refers to the company awarded this contract and all of the company's employees/subcontractors who perform services under this contract. b.FTEE - Full Time Equivalent Employee. c.1 FTEE - Equals the number of hours on Full Time Equivalent Employee is expected to work each year, which is 2,080 hours per year or 80 hours every two weeks. This number is not inclusive of Federal Holidays. A. SPECIFIC DUTIES AND RESPONSIBILITIES: 1.Comprehensive office visits to include initial patient evaluation as well as follow-up 2.Inpatient cardiology consultation and follow-up 3.Pacemaker insertion (temporary) 4.CVP (Central Venous Pressure) 5.Swan Ganz Insertion 6.Electrical Cardioversion 7.Transesophageal Echocardiogram 8.EKG (as requested by the medical center on an as needed basis). Fax, read and return within VAMC determined timeframes. 9.Cardiovascular stress testing 10.Arterial Doppler 11.Carotid Doppler 12.Holter Monitor 13.Event Recorder 14.On-call will be provided 24/7, 365 days a year. B. MISCELLANEOUS DUTIES AND RESPONSIBILITIES: a.Individual will follow and meet all medical center policies, rules, and regulations, including the Altoona VAMC's Medical Staff Bylaws (Attachment C). b.Individual will participate in organization meetings as requested. c.Maintains patient confidentiality and security of operations. d.The medical center utilizes a Computerized Patient Record System (CPRS). The Cardiologist must be familiar with personal computers, have adequate typing skills to perform data entry (progress notes, Cardiologist orders, and clinical reminders), and be able to navigate through windows based computer programs. Training on the Computerized Patient Record System (CPRS) will be provided by the medical center. All documentation entries will be made electronically in the Computerized Patient Record System (CPRS). All CPRS entries will be timely, accurate, and will be in accordance with VHA and VAMC rules and regulations. C. ADMINISTRATIVE DUTIES a. Patient Care 1. Cardiologist shall comply with VA Performance Measures and Monitors. 2. Cardiologist shall perform medical record documentation. 3. Cardiologist shall sign medical record immediately after encounters with patients. 4. Cardiologist shall ensure electronic informed consent is signed. 5. Cardiologist shall ensure timely completion of electronic encounters. 6. Cardiologist shall ensure timely completion of consults. b. Attendance 7. Cardiologist shall be present for regular scheduled hours. c. Quality Care 8. Cardiologist shall respond to quality care concerns. d. Patient Safety 10. Cardiologist shall respond to patient safety incidents or close calls. e. Patient Satisfaction 11. Cardiologist shall respond to patient complaints about the quality of care. 12. Cardiologist shall be familiar with patient rights and responsibilities. f. Education 13. Cardiologist shall complete all required training, education and competencies to maintain privileges. g. Interpersonal and Communication Skills. 14. Cardiologist shall demonstrate evidence of interpersonal and communication skills. D. QUALIFICATIONS Personnel assigned by the Contractor to perform services covered by this contract shall have a full and unrestricted license in a State, Territory, or Commonwealth of the United States or the District of Columbia. Contractor personnel assigned to perform services on by this contract shall be proficient in written and spoken English (38 USC 7402). Contractor's assigned personnel must be computer literate, have adequate typing skills to perform data entry, and be able to navigate through laboratory related computer programs. The qualifications of such personnel shall also be subject to review by the VA Chief, Acute Medicine and Chief of Staff and approved by the VA Facility Director. The Contracting Officer will be notified prior to any changes in personnel. VA reserves the right to approve the assignment of individual personnel furnished by the contractor to perform the functions specified in the Contract. At a minimum the contract Cardiologist must be board eligible in Cardiovascular Disease.
