- The VA Healthcare Network Upstate New York has a requirement for artificial limb fabrication, orthotic appliances and related services for the Syracuse VA Medical Center. The service area is Oneida, Madison, Herkimer, and Lewis Counties. Contractor performance shall include fabricating all artificial limbs in strict accordance with the VA prosthetic prescription provided by the VA Amputee Clinic Team and orthotic appliances in strict conformance to the orthotic prescription that has been submitted by the designated VA requesting provider. The VA shall require the Contractor to deliver items ordered under the awarded contract(s) no later than 30 days after receipt of delivery order. The Term of the contract shall be one (1) base period, with one (1) one year option renewal. The VA anticipates award of one or more contracts from this solicitation and will award a contract to responsible offerors whose offers are conforming to the solicitation and will be most advantageous to the Government, price and other factors considered. The preferred method of payment will be the Government Purchase/VISA Credit Card and the Contractor must be capable of accepting it for payment. This procurement is 100% small business set aside. The North American Industry Classification System (NAICS) code for this procurement is 339113 and the size standard is 500 employees. This solicitation adheres to the format defined in FAR Part 12 for the acquisition of commercial items.
