- Attachment #7 - FA5004-12-R-0001-0002 - Q&A #1 Amendment #2 - FA5004-12-R-0001-0002 - SF 30 Modification document Eielson AFB, Alaska has a requirement for Mess Attendant Services. Except for items specified in the Performance Work Statement (PWS) and Quality Assurance Surveillance Plan (QASP) as government furnished property or services, the contractor shall provide all personnel, supervision, any items and services necessary to perform food service attendant services at the Two Seasons Dining Facility at Eielson AFB. The request for proposal number is FA5004-12-R-0001 and will be made available to be downloaded at www.fedbizopps.gov on or about 23 December 2011. This action is 100 Percent (100%) small business set aside in accordance with FAR 19.502-2(a)(b)(c). The applicable North American Classification System (NAICS) code is 722310, with a small business size standard of $35.5M. This solicitation will be issued electronically. It will be available only via the Federal Business Opportunities web site (www.fedbizopps.gov). Potential offerors are responsible for downloading their own copy of the solicitation package, as no paper copies will be furnished. No bidder mailing list will be maintained. Potential offerors should register at www.fedbizopps.gov in order to receive notification and/or changes to the solicitation. Interested contractors are encouraged to submit their contact information by registering as an interested vendor utilizing the Vendor Notification Service link within the Register to Receive Notification function. All Offerors MUST be registered in the Central Contractor Registration (CCR) database. Offerors may obtain information on registration and annual confirmation requirements via the Internet at http://www.ccr.gov or by calling 1-888-227-2423 or 269-961-5757. Offers will be submitted on the Standard Form 1449 and must include as a minimum the items listed in FAR Clause 52.212-1 (b) (1-11) Instructions to Offerors - Commercial Items. This clause may be viewed at http://farsite.hill.af.mil. Any questions concerning this solicitation MUST be submitted in writing (mail, fax or email) to the point of contact listed below. See Addendum to 52.212-1 for methods of submitting proposals contained with amendment #2. Offers must be received no later than the date listed in the solicitation (subject to change).
