- This is a request for quote for repair of Farmington, Missouri VOR site. Vendor will provide all materials, supervision, labor and equipment to perform miscellaneous repairs according to the attached Scope of Work. Site is located in St. Francois County, Missouri. The FAA will not pay for any information received or costs incurred in preparing the response to the request for quote. Therefore any cost associated with the request for quote submission is solely at the interested vendor's expense. The competition will be based on lowest price. This competition is open only for small businesses, service-disabled veteran-owned small businesses or eligible socially and economically disadvantaged businesses that are certified by the SBA for participation in the SBA's 8(a) Program. Contractors must be registered in the Central Contract Registry (https://www.bpn.gov/ccr/default.aspx) prior to submitting a response. A pre-bid site visit will be conducted at 9:00 am, Wednesday, February 1, 2012. Contractors must notify Anthony Ghoston, 314-890-7213, by COB Tuesday, January 31, 2012 to have their name placed on the access roster. Since the project location is in a restricted area all contractors must be registered. This will be the only site visit allowed. It is highly recommended interested parties attend this site visit to develop a more responsive quote. Interested firms are required to submit the attached Business Declaration form and a copy of their SBA 8(a) certification letter, if applicable. In order to make this determination the FAA requires the following from interested vendors: 1. Pricing, utilizing the attached cost break-out sheet. 2. Capability Statement - This document should identify: Type of services provided by your firm, Size and type of services similar to this RFQ provided by previous contracts (elaborate and provide detailed information and past performance), Number of years in business Attached are the Scope of Work, Terms and Conditions, Bid sheet, and a Business Declaration Form. A Wage Determination has been requested from DOL for this work. Please respond by reviewing these items, and submitting a completed, signed pricing sheet, capability statement and a filled-out and signed copy of the Declaration form (please submit electronically, email to address below). All responses to this request for quote must be received by Thursday, February 16, 2012, 3:00 CST (Oklahoma Time).
