VBH130502 Viabahn Stent 1 EA Notice to Contractor: Solicitation number N00259-12-T-0654 is issued as a notice to procure from only one source IAW FAR 13.106-1(b). This proposed contract action is used for Radiology Department for Viabahn Stent for the Patient with severe vascular compromise. The Naval Medical Center San Diego intends to solicit with only one source under the authority of FAR 13.106-1(b). The source is W.L. Gore & Associates, Inc. CAGE Code: 1BX19, NAICS: 339113. Justification: As required by FAR 13.106-1(b), the Contracting Officer has determined that W.L. Gore is the only company that makes Heparinized Stent and company that manufacture and distribute a heparinize bioactive surface stent. Our Neurosurgeon has been trained on this specific instrumentation system and it is the one they feel most comfortable using on this particular procedure. IAW FAR 5.207(c) (15) (ii), all responsible sources may submit a quotation, which shall be considered by the agency. The Government will only consider firm, fixed price quotations. The attached solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-55. It is the contractor ™s responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at www.acquisition.gov and/ or www.acq.osd.mil.
