- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 and is supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation is attached. Solicitation number VA248-12-Q-0647 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Regulation (FAR) and Veteran Administration Acquisition Regulation (VAAR). The proposed acquisition is restricted for Nonprofit 501 (c)(3) agencies within the New York State area. Additional solicitations may be necessary in other states depending on the need. The need is driven by the number of approved applicants participating in the study and their geographical location. The associated NAICS code is 812910, with an applicable size standard of $7.0m. The Department of Veterans Affairs, VISN 8/NCO 8, Tampa, FL, has a requirement for Service Dogs to include Dog Health Insurance in accordance with the Statement of Work and Public Law (P.L.) 111-84, U.S Senate Bill S.1390, Section 1077 as attached to this solicitation. The Government anticipates award of a Firm Fixed Price Blanket Purchase Agreement (BPA). This RFQ will be evaluated in accordance with 52.212-2 within the attached solicitation. The Government intends to award one or multiple BPAs as a result of this solicitation; however, the Government reserves the right to award no BPAs at all, depending on the quality of offers submitted. An offer will be considered unacceptable and will not be considered for award if technical acceptability cannot be met. The attached solicitation VA248-12-Q-0647 includes FAR and VAAR provisions, clauses, Statement of Work (SOW), and evaluation criteria that apply to this acquisition. An offeror shall complete only paragraph (k) of 52.212-3 provision if the offeror has completed the annual representations and certificates electronically at http://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (b) through (i) of this provision. Offeror must be registered in the Central Contractor Registration (CCR) database before an award can be made to them. If the offeror is not registered in the CCR, it may do so through the CCR website at http://www.ccr.gov Proposals are due no later than 4:00 p.m. EST on January 23, 2012 and must be sent electronically to the individuals listed below.
