AMENDMENT NOTICE:This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; bids are being requested and a written solicitation will not be issued.The solicitation number is VA0240-12-Q-0018 and is issued as an invitation for bids (IFB), unless otherwise indicated herein.The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-55. The associated North American Industrial Classification System (NAICS) code for this procurement is 334517 with a small business size standard of 500.00 employees.This requirement is unrestricted and only qualified offerors may submit bids.The solicitation pricing on www.FedBid.com will start on the date this solicitation is posted and will end on 2012-01-17 14:00:00.0 Eastern Time or as otherwise displayed at www.FedBid.com.FOB Destination shall be Brooklyn, NY 11209 The VHA SAO - 240E - Contracting Office requires the following items, Brand Name or Equal, to the following: LI 001, Review Operators Log Book, Travel and Labor For Clean and Disinfect, Materials and Chemicals, Prefilters for Central and 3 WRO's, Cleaning and Disinfecting 3 WRO's tanks * THIS WILL BE A MONTHLY SERVICE, 9, EA; LI 002, De-Ionization(DI)Exchange, Carbon Rental 8x18, includes initial set up and quarterly exchanges of WRO's tanks (3)* *THIS WILL BE A QUARTERLY SERVICE, 4, EA; LI 003, Association for the Advancement of Medical Instrumentation(AAMI) Test for 4400 and 3 WRO's*THIS WILL BE A SEMI-ANNUAL SERVICE, 2, EA; LI 004, Tank Vent Filter Replacement, Loop Filter Replacement, RO(Reverse Osmosis) Membrane Replacement, Post DI Filter Replacement, Replace Membranes in 3 WRO's* THIS WILL BE A ANNUAL SERVICE, 1, EA; LI 005, **Emergency Response time: Contractor shall be return a phone call from VA Medical Center with a qualified technician within 8 hrs of a service request and an if it is qualified as a emergency request, will have an onsite response by a qualified technician in less than 24 hrs of a original request for emergency service. Qualified Emergency calls will be made by COR or their assigned representative and a visit will be set up by Contractor and their technician within an 8 hour window of initial call. Contractors? Technical service personnel are required to sign in and out of the Government facility., 1, JOB; Solicitation and Buy Attachments ***Question Submission: Interested offerors must submit any questions concerning the solicitation at the earliest time possible to enable the Buyer to respond. Questions must be submitted by using the 'Submit a Question' feature at www.fedbid.com. Questions not received within a reasonable time prior to close of the solicitation may not be considered.*** For this solicitation, VHA SAO - 240E - Contracting Office intends to conduct an online competitive reverse auction to be facilitated by the third-party reverse auction provider, FedBid, Inc. FedBid has developed an online, anonymous, browser based application to conduct the reverse auction. An Offeror may submit a series of pricing bids, which descend in price during the specified period of time for the aforementioned reverse auction. VHA SAO - 240E - Contracting Office is taking this action in an effort to improve both vendor access and awareness of requests and the agency's ability to gather multiple, competed, real-time bids.All responsible Offerors that respond to this solicitation MUST submit the pricing portion of their bid using the online exchange located at www.FedBid.com.
