- 52.219-1 Small Business Program Representations. 52.211-6 Brand Name or Equal. 52.204-3 Taxpayer Identification. This is a procurement conducted pursuant to the authority of Federal Acquisition Regulation (FAR) Part 13, Simplified Acquisition Procedures. The Billings Area Indian Health Service (IHS) intends to award a fixed-priced, purhase order in responce to Request for Quotation (RFQ)-10-12-032-JH. The solicitation is restricted to 100% Small Business concerns. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acqusition Circular2005-54. The associated North American Industry Classification System code is423450 and the small business sizestandard is 100 employees. The Billings Area, Indian Health Service is is requesting quotes for one(1) Optovue or Equal Ocular Coherence Topographer-must be spectral domain OCT- have retina, optic nerve and anterior segment analysis capabilities prefer unit that is mobile and equipment needed to be able to transport and function in second location. One (1) Optovue or Equal - ivue-complete Configuration 120V - Scanner head, Cotrol Box-Labtop, Labtop Case - Mouse- Footswitch- Can Lens - Can Box- Cables - Joystick, chinrest assembly, Cradle and User Manual. One (1) Optovue or Equal Travel Case. One (1) Optovue or EqualIvue-XYZ base assembly-joystick chinrest ass. One (1) Optovue or Equal Ivue-Motorized Table 12 volt. One (1) Optovue or Equal - Kit iVue 3D & Gcc map Upgrade USCFG.
