- HSCEMS-12-Q-00004 Attachment 3 Past Performance Questionnaire HSCEMS-12-Q-00004 Attachment 2 Pricing Template HSCEMS-12-Q-00004 Attachment 1 Statement of Work HSCEMS-12-Q-00004 Solicitation HSCEMS-12-Q-00004 SF-1449 This acquisition is being solicited in accordance with FAR Part 12 -Acquisition of Commercial Items in conjunction with the policies and procedures for solicitation evaluation, and award prescribed in FAR Part 13 -Simplified Acquisition Procedures, as appropriate for this particular acquisition. This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Part 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation is attached. Solicitation Number HSCEMS-12-Q-00004 is issued as a Request for Quotation (RFQ) for College Level Accounting Courses in accordance the Statement of Work (SOW) in Attachment I. A statement that the solicitation document and incorporated provisions and clauses are in effect through Federal Acquisition Circular Number 2005-55, dated January 3, 2012. The U.S. Department of Homeland Security (DHS), U.S. Immigration and Customs Enforcement (ICE) anticipates issuing a Firm Fixed Price Purchase Order. The NAICS code for solicitation HSCEMS-12-Q-00004 is 611310 for Colleges, Universities, and Professional Schools. NAICS Code 611310 has a size standard of $7.0 Million. HSCEMS-12-Q-00004 is an unrestricted solicitation. The Contract Line Items under RFQ No. HSCEMS-12-Q-00004 are as follows: Item No. Schedule of Supplies/Services/Quantity/ Unit CLIN 0001, Accounting Class: Base Period, February 1, 2012 - June 30, 2012, 6 Months CLIN 1001, Accounting Class: Option Period I, July 1, 2012 - December 31, 2012, 6 Months CLIN 2001, Accounting Class: Option Period II, January 1, 2013 - June 30, 2013, 6 Months CLIN 3001, Accounting Class: Option Period III, July 1, 2013 - December 31, 2013, 6 Months CLIN 4001, Accounting Class: Option Period IV, January 1, 2014 - June 30, 2014, 6 Months CLIN 5001, Accounting Class: Option Period V, July 1, 2014 - December 31, 2014, 6 Months The vendor shall furnish all personnel and services and otherwise do all things necessary to or incidental to performing and providing services described in the Statement of Work (Attachment 1). A FAR 52.212-1 Instructions to Offerors-Commercial, applies to this acquisition and the addenda to the provision. FAR 52.212-1, addenda and FAR 52.212-2, Evaluation - Commercial Items is tailored and, 52.212-3 Offeror Representations and Certifications-Commercial Items are included under RFQ No. HSCEMS-12-Q-00004 is attached. FAR 52.212-2, Evaluation - Commercial Items is applicable to this acquisition, is tailored, and is contained in the attached RFQ No. HSCEMS-12-Q-00004. Vendors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with their quotation submission. FAR 52.212-4, Contract Terms and Conditions-Commercial Items, apply to this acquisition and is included by reference under RFQ No. HSCEMS-12-Q-00004. FAR 52.212-5,Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items applies to this solicitation. Additional FAR provisions and clauses are included by reference and in full text in RFQ No. HSCEMS-12-Q-00004 that are applicable to this acquisition.
