- Schedule of Items & Performance Work Statement Wage Determination Building Floor Plan Experience & Past Performance Questionnaire Form First page of solicitation, SF 1449 AG-84N8-S-12-0011 - Solicitation for Janitorial Services on the Fishlake National Forest, for the Fillmore Ranger District facility in Fillmore, Utah. This combined synopsis/solicitation is issued as request for quotation (RFQ). This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, Stream-lined solicitation for commercial items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Forest Service contemplates award of a firm-fixed-price contract with 1 base year and four (4) options years as a result of this combined synopsis/solicitation. Award will be made using Simplified Acquisition Procedures, therefore the evaluation procedures at FAR 13.106-2 will be used. A firm-fixed price award will be made to the responsible offeror submitting the proposal that provides the best value to the Government. Best value will be determined by using the trade-off process as listed in FAR Part 13. In determining best value, the following factors will be considered: 1) Technical Capacity 2) Past Performance 3) Price Technical and past performance, when combined, is significantly more important than price. This request for quote is being issued pending availability of funds; this solicitation/contract is conditioned up the passage of an appropriation by congress from which expenditures there under may be made and shall not obligate the United States upon failure of congress to appropriate funds. There is no guarantee that any contractor awarded the base will be awarded any option year period. The Government will consider use of the option period based on continued need for service, available funding and contractor performance. The proposed contract is 100% set aside for small business concerns. All responsible small businesses may submit a proposal which shall be considered by the Forest Service (FS). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-46. Solicitation number is AG-84N8-S-12-0011. This solicitation is issued as a request for quote. The NAICS Code is 561720; Small business size standard is $16.5M. This requirement consists of general janitorial services. The Contractor must provide all management, supervision, personnel, transportation, equipment, supplies and labor that are necessary to perform the janitorial services at the Fishlake National Forest, for the Fillmore Ranger District Office, Fillmore, Utah. The requested janitorial services are defined in the Description/Specifications. The services requested must be in strict accordance with 1) Best industry standards and codes; 2) Federal, State, and local regulations; 3) Federal environmental standards; and 4) OSHA regulations. All proposals and required documentation shall be submitted to the Forest Service, Utah Acquisition Support Service Office, Attention Carmel Hall, 2222 West 2300 South, Salt Lake City, Utah 84119, no later than January 30, 2012 at 4:30 p.m. Mountain Standard Time. Include the solicitation number on the envelope or the fax cover page. E-mail quotes will not be accepted.
