Description
Statement of Work and Terms and Conditions SF1449 Cover Letter This is a combined synopsis/solicitation for the acquisition of commercial items prepared in accordance with the Federal Acquisition Regulations (FAR) Part 12 and Part 13, as supplemented with additional information included in this notice. The Social Security Administration (SSA) in the New York Region 2 has a requirement for licensed physicians as contractors to provide medical consultant services in a variety of medical specialties including but not limited to psychiatry, internal medicine, and orthopedics. SSA's Center for Disability utilizes the services of physicians in review of disability claims. Services will be performed on site at the Javits Federal Building in lower Manhattan, NY or the the Addabbo Federal Building in Jamaica, NY. Competitive pricing will be solicited and accepted. Consultant services will include providing oral and/or written consultations resulting from reviews of SSA disability case files, consulting with SSA medical policy staff and/or mentoring other medical consultant contractors. The contractor must have the necessary computer skills to read electronic case files, complete electronic forms, use electronic medical and SSA policy reference material, read and answer email messages, type narrative consultations and input electronic case information. No patient contact will be involved. New medical consultants (those not currently under contract to provide these services to the SSA) will be required to participate in an orientation program that is intended to enable the contractor to attain and achieve independent status as an SSA medical consultant. The period of performance for the BPAs is date of award through December 31, 2016. The government intends to award multiple Blanket Purchase Agreements (BPAs) for medical consultant services as a result of this solicitation. The Solicitation Number is SSA-RFQ-R02-12-1004. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-54. The associated North American Industrial Classification Code for Physicians Offices (except Mental Health) is 621111, with a small business standard of $10 million and for Physicians Offices (Mental Health) 621112, small business standard $10 million. This requirement is a 100% Small Business Set Aside. Offers received from other than small business concerns will not be considered. This acquisition is conducted in accordance with the Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, and FAR Part 13, Simplified Acquisition Procedures. The selected offeror must comply with the following commercial item terms and conditions as well as any addenda to these clauses. The FAR clauses and provisions that apply to this acquisition are FAR 52.212-1, Instructions to Offerors - Commercial Items, FAR 52.212-4, Contract Terms and Conditions - Commercial Items. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items. In addition, offerors must submit a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. Applicable terms and conditions, and representations and certifications are attached. Please be advised that prospective offerors may access www.bpn.gov to complete Online Representation and Certifications (ORCA). The Government will award BPAs resulting from this solicitation to the responsible offeror(s) whose offer conforms to the requirements of this solicitation. To be eligible for an award, Offerors must be registered in the Central Contractor Registration (CCR) database. If the offeror is not currently registered in the CCR, it may do so through the CCR website at http://www.ccr.gov. Offerors must have a Dun & Bradstreet (DUNS) number to begin registration. All award recipients will be required to undergo a security background screening which before beginning performance. Quotations in response to this combined synopsis/solicitation must be received by 2:00 p.m. Eastern Time on February 15, 2012. Offerors may send responses via email to Angela Lee, Contract Specialist, at Angela.Lee@ssa.gov. All questions regarding this RFQ must be submitted in writing at the email address Angela.Lee@ssa.gov. The Government will make every attempt to answer questions which will not automatically result in the extension of the proposal due date. All questions and their respective answers may be made available to all prospective offerors in the form of solicitation amendments. A pre-quote conference is scheduled for January 31, 2012 at 10:00 AM Eastern Time, 26 Federal Plaza, 40 Floor, New York, NY 10278.
