- CLIN DETAILS (i) The proposed contract action is for commercial item, which the U.S. Naval Hospital Okinawa intends to solicit this requirement on a sole source basis to Depuy Orthopaedics under the authority of FAR 13.106-1(b). This is a combined synopsis/solicitation prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested. Anticipated award date is 28 Dec 2011. (ii) This solicitation/synopsis reference number N68470-12-T-0207 is issued as a Request for Quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-54 dated 02 Dec 2011. (iv) NAICS Code for this requirement is 339113. (v) Contractors shall submit a quote for DVR Anatomic Volar implants. The deadline for questions pertaining to this requirement is 1200 PM, JST, 20 Dec 2011. See attachment for the Schedule of Supplies. Under the authority of FAR 13.106-1(b), sole source justification is based on Depuy Orthopaedics. being the only company supplying these particular items and implants for special type of orthopedic musculo-skeletal injuries that require surgery which are continually utilized at USNH Okinawa. Consumable work in conjunction with existing government owned equipment and instrumentation. (vi) Contractor must be registered in Central Contractor Registration, FAR 52.204-7 [Note: Lack of registration in the Central Contractor Registration will make a Contractor ineligible for award] Registration is available via the Internet at www.ccr.gov. Delivery FOB Destination to United States Naval Hospital (USNH)- Okinawa, Japan BLDG 6000 Camp Lester Chatan Cho Kuwae, Okinawa, Japan 904-0103 by 31 Jan 2012..
