- BPA Health Assessment The New Jersey Army National Guard is planning to establish a blanket purchase agreement for medical screening services. Annually, approximately 250 personnel will require a series of OSHA mandated medical examinations. The types of exams required will be determined by the nature of the working environment of the employee. All examination will be conducted at the chosen contractors' medical facility. Typical examination may include DOT physical exam, Back assessment, vision, audiogram, pulmonary function test, respirator fit test, chest X-ray, EKG, Lipid panel, PSA, CBC, urinalysis and blood lead, with additional test as required. This is a sources sought notification, not a solicitation. The intent is to identify potential vendors while Statement of Work is defined. It will be solicited as a small business set aside.
