Description
SOURCES SOUGHT NOTICE: The Department of Veteran Affairs NF/SG VHS Facilities within the Veterans Integrated Service Network (VISN 8) is seeking responsible sources for a Preventative maintenance and repairs to a Toshiba, Aquilion One CT Scanner, as outline in the Statement of Work. This is a market research for informational and planning purposes only. THIS IS NOT A SOLICITATION AND DOES NOT OBLIGATE THE GOVERNMENT TO ISSUE A SOLICITATION. Please be advised that the Government will not pay for any information submitted under this announcement. Your response should identify your company name, point of contact with contact information, indicate all areas you are capable of providing service, capabilities statement and identify your business size, and if you are (by the definitions in the FAR) any of the following: Small Business, 8(a) disadvantaged, disadvantaged, woman-owned, veteran-owned, service disabled veteran owned or HUB Zone certified. Responses to this notice shall be e-mailed or faxed to Shirley Courtney, Contract Specialist, FAX 352- 381-5746; E-mail address: shirley.courtney@va.gov. Telephone responses will not be accepted. Responses must be received in writing no later than 3:00 p.m. EDT December 30, 2011. Responses should clearly state their ability to provide the services. If a solicitation is issued, it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from their responses to this announcement. Statement of Work Background: The Malcom Randall VA Medical Center at 1601 SW Archer RD, Gainesville, FL maintains an annual service contract on Toshiba medical equipment. Objective: This service contract is an annual preventative maintenance and service contract on Toshiba medical systems Scope of Work: Scheduled visits to conduct normal preventive maintenance and repair to the Toshiba medical equipment. "Toshiba Aquilion One CT Scanner "All glassware Data/Deliverables "Maintain a uptime commitment of 98% "Standard 30 minute phone response "Standard 4 hour onsite response "Provide remote technical assistance to repair problems in the system "Modify software programming during schedule visits to resolve problems encountered by the Operators "Perform Software Analysis & Optimization each site visit "Automation Control Analysis & Optimization every year "Operator Training as necessary during scheduled visits "Firmware Updates performed to keep the system operating with the latest software "Provide regular software updates "Software Backups are to be performed whenever system modifications are made "Visual inspection of electronic components "Visual inspection of electrical components "Written report of all findings and recommendations provided to customer within five business days. Qualifications/Certifications "The contractor shall be qualified and have the required levels of professional and technical experience to perform the work required by this statement of work. "The contractor and all of the individuals working on the project site must have taken the OSHA Certified 10-hour construction safety course. Safety Regulations "All applicable safety regulations shall be observed during all work of this contract. "The VA Safety Officer shall have full authority to see that the contractor obeys all safety rules and regulations relative to the fulfillment of this contract. Scheduling and Hours of Work "All work in areas involved shall be scheduled with the VA Operations Supervisor. "Work shall be scheduled for proper execution to completion of this contract. "Field engineer onsite coverage will be Mon-Fri 8:00am-9pm and Preventative Maintenance will be done covered Mon-Fri 8:00am-9pm. Evaluation/Selection Factor: This service contract must be performed by a field service engineer that has the technical skill and ability to maintain and operate Toshiba medical systems. Service call labor rates for service contract customers should be discounted.
