- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W91243-12-T-0002 is being issued as a Request for Quotation (RFQ). W91243-12-T-0002 incorporated provisions and clauses are those in effective through Federal Acquisition Circular 05-54. This is set-aside for small business with a NAICS Code of 621999. CLIN 0001: Medical Surveillance Exams (See attached Statement of Work). This service will be held approximately the last week of January 2012. The date will be made at the time the contract is awarded. There will be two locations for these medical exams. One location will be in Lincoln Nebraska at the CSMS, 2960 North Park Road, Lincoln, NE 68524 and the other at the Greenlief Training Site, 6698 East "J" Street, Hasting, NE 68901. Payment will be done by one (1) Lump Sum Payment via Wide Area Work Flow (WAWF). This information will also be provided at the time is contract is awarded. The following provisions and clauses are applicable to this RFQ: 52.212-1 - Instructions to Offerors-Commercial Items; 52.212-2 - Evaluation-Commercial Items (Lowest price that provides all required medical services); 52.212-3 - Offeror Representations and Certifications-Commercial Items (must be included in pricing); 52.212-4 Contract Terms and Conditions - Commercial Items; 52.212-5 - Contract Terms and Conditions Required to Implement Statutes Of Executive Orders-Commercial Items. One lump sum payment paid via Wide Area Work Flow (WAWF) will be made on this contract. Pricing is due no later than 10:00 a.m. CST, Wednesday, December 28, 2012. Pricing will be accepted via e-mail to sandra.leach2@us.army.mil or fax to 402-309-7549. If you have questions, please e-mail them directly to me and I will get answers from the end user.
