Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. DESCRIPTION: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; bids are being requested and a written solicitation will not be issued. The solicitation number is: W912PF-12-T-0002 Facilities, Equipment, and Ground Maintenance - Army Field Support Battalion - Italy, USAG Livorno, Italy; and is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-54. The associated North American Industrial Classification System (NAICS) code for this procurement is 561210. The Government contemplates a firm-fixed price commercial item contract award. The 409th CSB RCO Vicenza, Suboffice Livorno requires the following services to be rendered: Line Item 0001: Monthly Facilities, Equipment, and Ground Maintenance for the Army Field Support Battalion - Italy at Leghorn Army Depot, USAG Livorno, Livorno, Italy, per the Performance Work Statement date 18 November 2011; quantity 6. Line Item 7500, Contract Manpower Reporting, Each, quantity 1: CONTRACT MANPOWER REPORTING REQUIREMENT: The Contractor is required to report all Contractor manpower (to include subcontractor manpower) required for the performance of this contract. The Contractor is required to completely fill in all the required fields in the reporting system using the following web address: https://cmra.army.mil/. The requiring activity will assist the Contractor with the reporting requirement as necessary. The Contractor may enter reports at any time during the reporting period, which is defined as the period of performance not to exceed 12 months ending 30 September of each Government fiscal year. Reporting must be completed no later than 31 Oct for every year or part of a year for which the contract is in place. Failure to comply with this reporting requirement will result in contract termination. Il rapporto dovra comprendere un periodo non superiore a 12 mesi con termine 30 Settembre di ogni anno fiscale, compilato in ogni momento del suddetto periodo, ma comunque sottomesso entro e non oltre il 31 Ottobre di ogni anno riguardante il periodo del contratto. La mancata complilazione del suddetto rapporto potra avere come conseguenza la chiusura del contratto per inadempienza. Period of Performance: This contract is for six (6) months. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications, if applicable; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items. The full text of the referenced FAR clauses may be accessed electronically at https://www.acquisition.gov/far/. The following also applies: FAR 52.236-1 Performance of Work by the Contractor (APR 1984) The Contractor shall perform on the site, and with its own organization, work equivalent to at least eighty (80) percent of the total amount of work to be performed under the contract. This percentage may be reduced by a supplemental agreement to this contract if, during performing the work, the Contractor requests a reduction and the Contracting Officer determines that the reduction would be to the advantage of the Government. All contractors must be registered in Central Contractor Registration (CCR) in order to receive a contract award. If the offeror is not registered in CCR, it may do so through the CCR website at http://www.ccr.gov. Evaluation Procedures to be used: The award will be made on the basis of the lowest evaluated price of proposals meeting the acceptability standards for the technical factor of Past Experience: This factor will be rated "ACCEPTABLE" based on whether the offeror demonstrates it has experience on at least three current or past contracts which are/were similar or greater in scope and complexity to the Government's requirement. The past contracts must have been completed within three years from the date of this solicitation or result in a "NOT ACCEPTABLE" Less than the required amount of experience and value will result in a "NOT ACCEPTABLE." Offerors must submit three completed forms of Attachment titled "Past Experience Questionnaire." A Determination of Responsibility will also be performed on the company's Past Performance based on the Past Experience Questionnaires and the company's CERTIFICATE OF THE CHAMBER OF COMMERCE (CERTIFICATO DI ISCRIZIONE ALLA CAMERA DI COMMERCIO) (ORDINARIO): The offeror shall provide with the proposal a copy certified and NOT older than two (2) months at the time set forth for the receipt of the offer of the Chamber of Commerce Certificate of Membership (Certificato di Iscrizione alla Camera di Commercio). Self-certifications are allowed. However, if the offeror has submitted a certified Chamber of Commerce Certificate of Membership to the issuing office (RCO SubOffice Livorno) within 90 days from the due date of this proposal, then the offeror may submit a copy of the previously certified Chamber of Commerce Certificate of Membership. The certificate must include the following statement otherwise the proposal will be rejected. "NULLA OSTA ai fini dell'articolo 10 della Legge 31 Maggio 1965, n. 575 e successive modificazioni. La presente certificazione e' emessa dalla C.C.I.A.A. utilizzando il collegamento telematico con il sistema informativo utilizzato dalla Prefettura di Roma."
