- DD 1423, Safety Plan Appendix C 'Industrial Safety and Health Requirements' Statement of Work This is a combined synopsis/solicitation for commercial items prepared IAW the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This annuoncement constitutes the only solicitation. Quotes are being requested, and a separate written solicitation will not be issued. This solicitation will not result in an order against a Federal Supply Schedule. Any offer which is predicated upon the award being placed against an FSS will not be considered. This requirement is not set aside for small business. ITEM DESCRIPTION QTY/UNIT AMOUNT 0001 YOUTH SOCCER OFFICIALS 1 LOT $ Non-Personal Services to include all Labor and uniforms necessary to Provide Youth Soccer Official services in accordance with the attached Statement of Work The pricing of officiating shall be in accordance with the following schedule: CLIN 0001 SOCCER LEAGUE PRICE PER GAME EST NO.OF GAMES TOTAL AMT U-8 $ 44 $ U-10 $ 33 $ U-12 $ 29 $ U-14 $ 23 $ U-16 $ 8 $ TOTAL CLIN 0001 PRICE:$ 0002 SAFETY PLAN 1 LO $ PREPARE SAFETY PLAN IAW ATTACHED DD FORM 1423 AND APPENDIX C Price should be FOB Destination (Robins AFB GA 31098). In addition to price, quote should include your cage code, DUNS number, and taxpayer ID number. AFFARS 5352.223-9001 - Health and Safety on Government Installations AFFARS 5252.242-9000 - Contractor Access to Air Force Installations In accordance with Clause 52.212-2, the evaluation factors shall be price and price related factors. NOTICE: The Total Price for each league will be determined by multiplying the price per game x the estimated number of games in each league. The sum of the total for each league plus the total for the safety plan equals the Total Evaluated Price. IAW FAR 52.212-3, Reps/Certs should be completed on-line at www.bpn.gov. Contractor must be registered in CCR and ORCA. Web site is www.bpn.gov. Contractor must be registered in Wade Area Workflow (WAWF) for invoicing. Email responses are accepted.
