Description
SOL: VA-261-12-R-0019 Due: Jan. 17, 2012 (est'd) Request for Proposal (RFP) - The Department of Veterans Affairs intends to issue a solicitation for 911 emergency ambulance services for the VA Pacific Islands Health Care System, Honolulu, HI. All indications are that this service can only be performed by the Department of Health, State of Hawaii. However this requirement must still be advertised should any interested offerors wish to inquire. The Product Service code is V225, the NAICS code is 621910; the size standard is $7.0 million. The actual solicitation should be posted on or about January 3, 2012. Telephone requests will NOT be accepted. All questions must be submitted in writing, questions may be faxed to W. Ulibarri, (650) 849-0395 or emailed to bill.ulibarri@va.gov. You may also mail to William Ulibarri, Network Contracting Office (NCO 21), VAPAHCS, Bldg. T-50, 3801 Miranda Ave., Palo Alto, CA 94304. Electronic solicitation may be accessed at no cost at www.fedbizopps.gov. Any amendment issued against this solicitation will also be available from the same URL addresses. Contractor is responsible for submitting all documents required in accordance with the solicitation, including any amendments which will be posted on this website. Solicitation is also subject to availability of funding. Proposals shall be submitted on an all or nothing basis. Partial quotations will be rejected. Proposals shall be valid for 60 days from offer due date. This solicitation and incorporated provisions and clauses are those in effect through FAC 2005-54. Offeror shall submit all information with offer as required in 52.212-1; a completed copy of provisions 52.212-3 Offeror Representations and Certifications. Offeror may obtain copies of the referenced provisions and clauses at http://arnet.gov/far and http://www.va.gov/oa&mm/vaar/ or by sending an email to the Contracting Officer at bill.ulibarri@va.gov. The Contracting Officer may add any additional contract requirements or terms and conditions (such as contract financing arrangements or warranty requirements) determined by the Contracting Officer to necessary for this acquisition and consistent with customary commercial practices. Proposals shall be submitted in electronic copy to: William Ulibarri, (NCO 21), VA Palo Alto Health Care System, Bldg. T-50, 3801 Miranda Ave., Palo Alto, CA 94304 or via fax to the Contracting Officer at (650) 849-0395. A hard copy may also be furnished but is not necessary. Web based offers will not be accepted.
