- This is a combined synopsis/solicitation for commercial items prepared IAW the format in FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for this requirement. This is a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the most recent Federal Acquisition Circular (FAC) and DFARs Change Notice (DCN). NAICS is 621399 and the size standard is $7,000,000 Annual Gross income. THIS ACQUISITION IS 100% SMALL BUSINESS SET-ASIDE. The Air Force Flight Test Center (AFFTC), at Edwards AFB, CA is seeking potential sources capable of providing Nurse Advice Line services in accordance with the attached Performance Work Statement(PWS). Period of Performance: 12 Months UNIT OF ISSUE: MONTHS QUANTITY: 12 Potential Option Period: 6 Months UNIT OF ISSUE: MONTHS QUANTITY: 6 INFORMATION CONTAINED IN THE PROPOSAL SHOULD INCLUDE MONTHLY COST, AND A FINAL BOTTOM LINE PRICE.
