Description
The Department of Veterans Affairs VISN 9 Acquisition Service Center, 1645 Medical Center Parkway, Suite 400, Murfreesboro, TN 37129 is soliciting on behalf of the VA Medical Center, 1030 Jefferson Avenue, Memphis, TN 38104 for a contractor to provide annual radiological testing. Contractor shall furnish all necessary travel, labor, materials, tools, documentation and parts required for the inspection, support, and repair of the equipment, software, and accessories listed in the Schedule of Equipment, located at the Veterans Affairs Medical Center, 1030 Jefferson Ave., Memphis, TN 38104. This contract shall cover the base period of November 22, 2011 through September 30, 2012, with the possibility of exercise of four (4) option years, in accordance with all terms, conditions, provisions, schedules, and specifications of this solicitation. Services to be performed by vendor who is board certified in Diagnostic Radiology by the American Board of Radiology or equivalent in the appropriate disciplines of radiologic physics, including diagnostic, therapeutic, and/or medical nuclear physics or equal who demonstrates competency in these disciplines. Certification is to be submitted with proposal. Service will be performed for the Veterans Affairs Medical Center (VAMC), 1030 Jefferson Avenue, Memphis, TN 38104. The Contractor shall apply established uniform safety and performance standards for diagnostic radiation machines, with the intent to minimize unnecessary radiation exposure to patients and allied medical personnel and to maximize equipment performance and image quality. Compliance with these standards will provide compliance with the Department of Veterans Affairs (DAV) M-2, Part XI, Manual, MP3 Manual (Part III, Change 1, Appendix 5M), U.S. Department of Health and Human services of the Food and Drug Administration (FDA) Regulations for the Administration and Enforcement of the Radiation Control for Health and Safety Act, National Council on Radiation Protection (NCRP) Reports 99 and 102, applicable State regulations, Accreditation Manual for Hospitals (JCAHO) and Occupational Safety and Health Administration (OSHA). Since the standards established herein are the minimum, for some specialized applications stricter or more comprehensive requirements may be appropriate. The Contractor shall furnish radiation compliance testing for the VAMC, which will include: Diagnostic X-ray radiation machine compliance testing (annually), a.The Contractor will perform x-ray radiation machine compliance testing annually per attached inventory in accordance with the Bureau of Radiological Health (BRH) requirements (see attached list of required test). Scheduling of compliance testing will be conducted at times to be mutually agreed upon by VAMC by Biomedical Engineering at extension 5240/5222. All tests will be performed with BMET present and the Contractor. Survey will be performed during the months of July, August or September. A written report of this survey and evaluation will be submitted to the VAMC Biomed, no later than 2 weeks after survey completion. For all equipment requiring corrective action, the VAMC Biomedical Engineering Section, Engineering Service (138) will provide the necessary repairs, adjustments or arrange to have qualified service personnel perform the required adjustments or repairs. b. If necessary, the Contractor will perform a follow-up survey ninety (90) days after the date of the written annual survey results and following major repairs or adjustments to verify all faults noted in the original survey or major repairs or adjustments have been corrected. Reported faults will contain test parameters and Biometry test setup for the purpose of duplication of results. c. Any employees of the Contractor that work on station must complete the VA Information Security Awareness and VHA Privacy Training prior to arriving on station. This training may be accessed via https://www.ees-learning.net. The contracting officer (COTR) shall be contacted if further instructions on completion of this training are needed. The Government intends to acquire a commercial item/service using FAR Part 12. The NAICS associated with this requirement is 541990 with a size standard of $34.5 Million. This is for full and open competition. It is anticipated that solicitation VA-249-12-Q-0014, will be issued on or around November 10, 2011 at http://www.fedbizopps.gov.
