- PWS - Performance Work Statement - LNDH Group Life Insurance USAFEI 36-752 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quote (RFQ). Funds are currently available for this requirement. This document and incorporated provisions and clauses are those in effect for Federal Acquisition Circular 2005-53. This acquisition is unrestricted. //Schedule of items: This requirement is for Group Life Insurance for six (6) Local National Direct Hire (LNDH) personnel at 426th ABS, Stavanger, Norway. Period of performance: 1 Jan 2012 - 31 Dec 2012. //CLIN 0001: Group Life Insurance for: Host Nation Advisor, Recreation Assistant, Financial Services Assistant, Contracting Specialist, Transportation Assistant. See attached Performance Work Statement. //CLIN 0002: Group Life Insurance for: Housing Management Assistant. See attached Performance Work Statement. IMPORTANT NOTES: 1. The contractor must show the cost for Housing Management Assistant as a separate item in the quote. 2. Contractor must specify on the invoice each item description, unit and price as it appears in the final contract. Invoice with contract number and amount in NOK must be sent to: Department of the Air Force, 426 ABS/FM, P.O. Box 8080 Eikesetveien, 4068 Stavanger, Norway. Upon receipt of invoice, it can take up to 45 days before payment is being made by DFAS (Defense Finance and Accounting Service) in Maine, USA. 3. Only quotes submitted in English will be considered. 4. Contractor must have a license to operate in Norway from The Financial Supervisory Authority of Norway in order to be considered for award (ref. USAFEI 36-752 section 15.8.). Personal information (i.e. names and date of birth of employees) will only be given to contractors with such a license. //When price quotations are submitted please note that quotes should be exclusive of Value Added Tax. OMBUDSMAN (AUG 2005) (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution.
