- Attachment E – Certificate of Current Cost or Pricing Data Attachment D_Non-Disclosure Agreement Attachment C_Past Performance Evaluation Survey Attachment B_HHS Section 508 Product Assessment template Attachment A_Wage_Determination Solicitation_#12-233-SOL-00024 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is 12-233-SOL-00024. The solicitation is issued as a request for proposal (RFP). (iii) A statement that the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53. (iv) This solicitation is 100% set-aside for Small Business 8(A), to be competed in accordance with FAR 19.805 Competitive 8(a). The corresponding NAICS code is 541511 and the small business size standard is $25Million. (v) A list of contract line item number(s) and items, quantities and units of measure, (including option(s), if applicable). - N/A (vi) The purpose of this Requirement is to provide the Program Support Center's (PSC) Business Office with functional & technical systems support of the PSC's Revenue Invoicing and Cost Estimation System (PRICES), the PRICES On-Line Viewer, the Fee-for-service Application for Monthly Planning (FAMP), and the Fee-For-Service Spreadsheet II (FFSS2). Additionally, contractor support is required to assist the PSC Business and Billing Offices develop business acumen and implement streamlined business processes that will benefit current and future PSC customers. Refer to Solicitation #12-233-SOL-00024 for further details. (vii) Date(s) and place(s) of delivery and acceptance and FOB point. Refer to Solicitation #12-233-SOL-00024. (viii) The provision at FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition. (ix) The provision at FAR 52.212-2, Evaluation - Commercial Items, applies to this acquisition. The Government will award a contract resulting from this combined synopsis/solicitation to the responsible offeror whose offer conforming to the solicitation will offeror the best value to the Government, price and other factors considered. (x) Offerors are to include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items, with its offer or acknowledge its listing in Online Representations and Certifications Application (ORCA). In order to receive an award, a contractor must be registered in the Central Contractor Registration (CCR) data base. (xi) The clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. (xii) The clauses at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition. Refer to Solicitation #12-233-SOL-00024 for the additional FAR clauses that are applicable to the acquisition. (xiii) Refer to Solicitation #12-233-SOL-00024 for additional contract requirement(s) or terms and conditions necessary for this acquisition and consistent with customary commercial practices.
