- PERFORMANCE WORK STATEMENT EQUIPMENT WAGE DETERMINATION SOLICITATION APPENDIX 'C' APPENDIX A Services Non-Personal: The Air Force intends to solicit full & open and issue a contract for the following services: The contractor shall provide all labor, tools, equipment, test equipment, materials, parts, transportation, and other incidentals necessary to travel to Tinker AFB, OK. to perform Preventive and Remedial Maintenance Support Services on 17 machines for 76PMXG and 5 machines for 76AMXG and 2 machines for 76CMXG for a total of 24 machines. These machines are in different buildings, Ten (10) are New Bold Embossing Tag Machines and fourteen (14) are Matica Embossing Tag Machines and the associated components. New Bold machines are Matica machine that marketed in the USA under New Bold brand name for a period. At the present time, four (4) of the machines are located in Building 9001, two (2) in Building 3705, two (2) in Building 3221, one (1) in Building 2122, one (1) in Building 3105, and the remaining twelve (12) are in Building 3001. The contractor shall be responsible for insuring all services, parts provided under this contract are in accordance with the terms, and conditions outlined herein and conform to the manufacturer's equipment specifications and this Performance Work Statement (PWS). Equipment per serial number and appropriate location is in Appendix A. A Firm Fixed Price (FFP) contract is contemplated for Preventative Maintenance and Time & Materials CLIN for the Remedial Maintenance. The proposed basic period of performance (POP) is: 26 Oct 2011 through 25 Oct 2012 The proposed first option POP is: 26 Oct 2012 through 25 Oct 2013 The proposed second option POP is: 26 Oct 2013 through 25 Oct 2014 The estimated award date is 25 Oct 2011. Central Contractor Registration (CCR) is required to receive any award. Contractors can register electronically at http://www.ccr.gov. ORCA registration is also required and contractors can register electronically at https://orca.bpn.gov. The NAICS code for this requirement is 811310 with a size standard of $7M.
