Description
The purpose of this Amendment 0002 is to extend the Response date and to answer the following questions in refferernce to The solicitation BPD-AFRW-12-CI-0002 Request for Proposal (RFP). New Response Date: Tuesday, October 18th, 2011 @ 2:00 pm ET. FBO Questions: Q4) Do you have historical utilization information that details the number of trips per month for the past Fiscal Year? (Wheelchair, ambulatory and stretcher, if possible) If not for the whole year, an average number of trips per month would be appreciated. Also, what percentage of the trips are subscription trips? (ie: scheduled every week - dialysis, chemotherapy, rehab etc.) ANSWER: Estimated average of trips will be approximately 17 per month with ambulatory/wheelchair utilization, Estimated 1 or less stretcher utilizataion per year. Currently two Resident transported to VA hospital for dialysis three time a week Q5) The AFRH Office closes at 4:00 pm each day. Where would the requests or after hours transport originate from? Also, who would assist with questions and/or issues after hours? ANSWER: 4 p.m. until 12 midnight: All transportation from 4 p.m. until 12 midnight is requested through the Wellness Center. The Wellness Center notifies the KHC Nursing Supervisor that transportation is required. The Request Form is prepared and faxed to the Secllrity Desk. Q6) The RFP states that the service is 24 hours/7 days per week. Can we propose an afterhours fee that will compensate for the "will call" requests that are not scheduled in advance? ANSWER: Yes Q7) Trips may require the driver to wait (per request from the AFRH office). Can we include a waiting fee? ANSWER: If the wait is for over 15 minutes, yes, but we have no records of waiting at this time. Q8) A "no show" charge is not listed in the solicitation, only cancellation fee. A "no show" would apply to the driver arriving at the pickup and the client is not there. Can a fee be assessed? ANSWER: Yes Q9) Due to the instability of the economy, there has been a need to include in contacts a fuel adjustment fee if fuel exceeds a predetermined cost. Can this be included in the proposal? ANSWER: Yes Q10) When can payment be expected? 30 days? 45 days? ANSWER: NET 30 Q11) Are the most common destinations the VA Medical Center and Walter Reed Medical Center? ANSWER: VA is the heaviest, and Washington, Bethesda (Walter Reed), Providence, and Silver Spring are the most common. But, we do go to multiple areas in the 30 mile radius. Q12) Will attendants be provided or will contractor have to provide one? If contractor has to provide, can we include a fee for this service? ANSWER: If an escort is required, AFRH will provide a nurse or another resident volunteer. Q13) Will contractor have to provide the wheelchairs? ANSWER: No, but may have to provide stretcher Q14) How much time will be allowed for startup prior to transportation services beginning? ANSWER: Anticipated award date is November 1, 2011, with full performance to begin no later then November 15, 2011.
