- Solicitation Package Cover Letter THERE IS NO SEPARATE REQUEST FOR PROPOSAL. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemental with additional information included in the attachment. This announcement constitutes the Request for Proposal. The Courts Services and Offender Supervision Agency has a requirement for Dual Diagnosis Outpatient Substance Abuse services. The Period of Performance will be from the Date of Award through September 30, 2012, with four one (1) year option periods. The solicitation number is CSOSA-12-0002. NAICS code for this procurement is 623220 with a size standard of $6M. See attachment for the following additional information: Instructions for completion of solicitation package, Schedule of Commercial Services and Prices, Statement of Work and the following clauses 52.212-1 Instructions to Offerors-Commercial Item, 52.212-3 Offeror Representations and Certification Commercial Item, 52.212-4 Contract Terms and Conditions-Commercial Items and 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items. Offers or Proposals will be evaluated in accordance with evaluation factors contained in the solicitation Evaluation Criteria. All questions regarding the RFP shall be submitted via email to Bobby.Jones@csosa.gov on or before October 17, 2011, 2:00 p.m. EST. Proposals are due on November 7, 2011 at 11:00 a.m. EST. Late proposals will not be accepted. Proposals shall be hand delivered to the Court Services and Offender Supervision Agency, Office of Procurement, 633 Indiana Avenue, NW, Suite 880, Washington, DC 20004. All proposals shall reference CSOSA-2-0002 on the front of the package/page. The request for proposals documented and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-50.
