Description
This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. We have a requirement for; Youth Ministry Services in accordance with The Performance Work statement as follows: PERFORMANCE WORK STATEMENT (PWS) For FY 2012 Youth Ministry Contractors 1. Description of Services. This PWS defines the Contractor requirements, government responsibilities, reporting, and control requirements related to youth ministry activities for Fort Riley, KS. Contractor shall provide youth ministry for youth, defined as Middle School and High School age students, within the Fort Riley community. Services rendered by the Contractor shall consist of providing a community-wide religious program, supporting specific youth ministry needs which cannot be provided by the Fort Riley Ministry Team or volunteers. It is the responsibility of the Contractor to articulate and develop promotional campaigns and endeavors, and to maintain records of supplies, materials and attendance of youth and volunteers. The major scope of work required shall focus on programming, developing and implementing activities for youth of the Fort Riley community in support of the established programmed services of the Fort Riley Ministry Team. 1.1. Requirements. The Contractor shall perform in accordance with this performance work statement. 1.2. The Contractor shall have specific training in youth work with a minimum of two years experience in youth ministry. The Contractor shall have received a specific orientation regarding the military community and military environment. A baccalaureate degree in Christian religious degree (e.g., Youth Ministry, Biblical Studies, Religious Education, or ministry) is preferred but not mandatory. The Contractor shall provide these records to the Government upon request prior to award of a contract. 1.3. The Contractor will submit requested information to the Garrison Chaplain in order that a background check may be conducted by the Government on all individuals, including volunteers, working with youth in conjunction with this contract. Failure to provide this information will result in the ineligibility of an individual to work with the youth. 1.4. The Garrison Chaplain may require the Contractor to remove personnel working under the contract for reasons of misconduct, security violations, or suspicion of being under the influence of drugs, alcohol or any incapacitating agent. The Contractor and/or its employees/volunteers shall be subject to dismissal from the premises upon determination by the Garrison Commander that such action is necessary in the interest of the government. The Garrison Commander has the authority under 10 U.S.C. 1382 to bar individuals from the Garrison. Removal from premises and/or bars from the Garrison shall not excuse the Contractor from its duty of contract performance. 1.5 The Contractor shall maintain strict confidentiality regarding persons counseled or interviewed and information relating to individuals participating in the Fort Riley youth ministry program. The Contractor shall not relate or provide sensitive information regarding persons, personal notes, checklists, or counseling records to unauthorized persons. 2. Specific Tasks. The Contractor has overall responsibility for ensuring that all programs and activities are inclusive of all Christian traditions. The tasks below are to be completed before 30 September 2012. Contractor shall: 2.1. Develop, coordinate, and conduct 75 youth group meetings for Middle School and/or High School students. Middle School and High School youth group meetings may be combined until such time that the Protestant Director of Religious Education and the Contractor agree that two separate meetings are necessary to better meet the spiritual needs of the students. These meetings may also include activities that are designed to reach out to Middle School and/or High School youth who do not normally attend a chapel. This meeting would also include hosting activities such as Bible Lunch Tuesday (B.L.T.), a fellowship and outreach activity to Junction City High School students. 2.2. Conduct two (2) service projects per year that are at least four (4) hours in duration, coordinated with and approved by the Protestant Director of Religious Education. Service projects may be for Middle School or High School or both, but each group shall have at least one opportunity to participate in a service project. 2.3. Conduct two (2) day long retreats/activities for Middle School and/or High School youth that are at least 12 hours in duration and include at least one over night stay. Contractor shall obtain written parental consent and medical release for participants in activities off Fort Riley, KS. 2.4. Make 80 casual student contacts per year during normal activities to strengthen or make new relationships. These are casual contacts at school or community events and activities with the intention of making contact with students but will generally not be to meet a particular student. Casual contacts should be at least ten (10) minutes in duration. This would include attending, but not hosting (see 2.1), Bible Lunch Tuesday (B.L.T.). The Contractor will provide a by name list of contacts made in the monthly report (see 2.10). 2.5. Make 10 student contacts per year to strengthen or make new relationships. These contacts are relational in nature in that the intention is for the Contractor to plan on meeting a particular student and spending at least 30 minutes with that student. The Contractor will provide a by name list of contacts made in the monthly report (see 2.10). 2.6. Provide four (4) recreational activities for fun, fellowship, and sharing for youth at least 6 hours in duration. An example of a day- long recreational activity would be a trip to a water or amusement park. Activities should be coordinated with and approved by the Protestant Director of Religious Education. Contractor shall obtain written parental consent and medical release for participants in activities off Fort Riley, KS. 2.7. Compile and prepare four (4) newsletters for publication. Newsletters should be published and delivered to the Garrison Chaplain's Office no later than the fifth day of January, April, July and October. The Garrison Chaplain's Office will be responsible for distribution of the Newsletter. Newsletters should include all youth ministry activities supported by the Garrison Chaplain for the upcoming quarter. 2.8. Conduct 50 Bible studies designed to develop personal growth in students. Bible study groups must include at least three students and 45 minutes of instruction. 2.9 The Contractor is required to submit a monthly significant activities report with each invoice. The report should include a brief description of the activity/event; the number of participants, number of volunteers and volunteer's hours and the duration of activity/event. 2.10 Coordinate with the Protestant Director of Religious Education and conduct a youth ski trip for Middle School and/or High School students. This trip will include activities that are designed to challenge and strengthen youth faith through fellowship, worship, devotional and Bible study activities. 2.11 The Contractor will assist the Protestant Director of Religious Education with recruiting, training, coordinating and managing youth volunteers in support of Fort Riley's Vacation Bible School (VBS). The Contractor will participate in at least two planning or volunteer training meetings, as well as participate for the entire VBS program. 2.12 Contract Support Mandatory Reporting Requirement. This is an annual reporting requirement in year for the service contract. The contractor shall visit the following web site https://contractormanpower.armv.pentagon.mil and answer the 13 questions. The cost of this requirement includes the costs associated with preparation of this report. This requirement shall be completed upon award of the contract. Requirement Summary. All work will be accomplished within the guidelines of Army Regulations and Directives, and the Garrison Chaplain's Policy Statements. AR 165 - 1, AR 165 - 18 and the Fort Riley Garrison Chaplain's Chapel Operations SOP dated 9 March 2011 apply. 4. Government Furnished Property and Utilities. The Government facilities and other materials necessary to implement their programs: 4.1. Facilities for appropriate administrative, planning, and group activities will normally be located in a Chapel. 4.2. Storage facilities for storage of all teaching materials and supplies. These storage facilities will normally be located on Chapel property. 4.3. Use of equipment to include projector, television, VCR/DVD player, and use of a copy machine and/or print shop. 4.4. Supplies to include paper products for publicity, markers, refreshments for events, and fuel for transportation in conjunction with approved youth ministry activities. 4.5 The Contractor shall provide a detailed list of all teaching materials and/or supplies to the Protestant Director of Religious Education for approval. 4.6. Limited use of Transportation Motor Pool (TMP) vehicles for the transportation of youth for specified events in conjunction with approved youth ministry activities. Requests shall be made through the Garrison Chaplain's Office (Garrison NCOIC). All requests are subject to approval of the Garrison Chaplain and dependent upon availability of vehicles and authorized drivers. Contractor shall comply with all TMP regulations, policies, complete all the government licensing requirements and have on their person a current government issued license for the appropriate vehicle. 4.7 Property Accountability. Contractor will be required to sign hand receipts for all issued Government property, including for any keys described in section 8 of this document. Any Government property lost or destroyed as a result of negligence on the part of the Contractor will be replaced by the Contractor at the current Government replacement cost of the property as determined by the Contracting Officer. 5. Contractor(s) - Furnished Items. The Contractor shall furnish all other required materials beyond what is stated as Government-furnished in section 4 above. 6. Quality Control. The Contractor, and not the Government, shall be responsible for all management and quality control actions to meet the terms of the contract. The role of the Government is quality assurance to ensure contract standards are achieved. The Contractor's quality control plan is the driver for product quality. The Contractor shall submit its QC program with proposal for evaluation. Once approved the Contractor shall not implement any Contractor changes to the approved plan until the revised/changed plan has been submitted to and reviewed and accepted by the KO. The plan at a minimum shall include: 6.1. The plan describes acceptable quality control techniques and process control methods tailored to each requirement in the contract (The plan include: checklists, inspection frequency system, sampling procedures as well as responsibility of personnel performing root cause analysis and process for responding to CDRs.) 6.2. The plan describes how these techniques and processes provide an effective measure of contract performance. 6.3. The plan includes specific inspection techniques and process control methods. 6.4. The plan includes techniques and methods consistent with quality concepts, and provides an effective measure of contract performance, encompassing management as well as technical performance to include a description of how performance will be measured against established standards for each requirement in the contract.. 6.5. The plan includes actions to be taken and methods to be applied in pinpointing problems or deficiencies in performance and for providing resolution and management initiatives to prevent their recurrence. 6.6. The plan describes control of documentation, reports and records as well as a system for product identification and traceability. 6.7. The plan includes a customer Interface, communication, satisfaction and feedback system. 6.8. The plan describes risk management and effective measures that will be in place to identify and prevent potential problems before they occur and methods to mitigate their effects. 6.9 The plan describes methods and processes to be employed in identifying and implementing opportunities for continuous improvement. 6.10 The plan describes adequate reports and records of corrective/preventive action and continuous improvement and follow-ups. 6.11 The plan identifies adequate resources to conduct quality control, the line of quality reporting, and the benefit derived from this line of report, and procedures for ensuring that the integrity of the quality control system is maintained acceptable. 7. Quality Assurance. The Government will monitor the Contractor's performance under this contract using quality assurance procedures. Under this contract the Protestant Director of Religious Education will serve as the monitoring official. Any substandard performance noted under the monitoring procedures will be reported to the Contractor for corrective action. 8. General Information. 8.1 Physical Security/Key Control. The Contractor bears the responsibility for physical security and key control under the provisions of this contract. The Contractor shall be responsible for keys provided by the government. Keys shall be safeguarded and not be duplicated. 8.1.1 The Contractor shall report any theft, loss, duplication or misplacing of keys to the government within one hour of the discovery of the occurrence. 8.1.2 The Contractor shall reimburse the government for replacement of keys and/or re-keying of locks as a result of keys being duplicated, lost or misplaced by the contractor or while under the responsibility of the contractor. 8.1.3 The Contractor shall be responsible for security of facilities and property made available by the Government in conjunction with the performance of this contract. 8.2Fire protection. Contractor shall comply with Garrison regulations for fire protection and fire prevention. 8.3Safety. Contractor shall comply with policies governing safety and smoking in applicable regulations to prevent accidents and preserve the life and health of the Contractor and of all persons associated in any way with the performance of this contract, as well as all health and immunization and environmental health requirements and provide/or maintain documentation. 8.4Contractor shall perform preventive maintenance checks on all vehicles prior to operating vehicles and transporting one or more youth and/or volunteers in conjunction with performance of this contract. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular Fac 2005-43, Effective 02 Aug 2010. This requirement is un-restricted. The applicable NAICS code is 813110 with a size standard of $7,000,000.00. There will be a total of 12 Contract Line Item Numbers, consisting of: CLIN 0001; Develop, coordinate, and conduct 75 youth group meetings for Middle School and/or High School students. CLIN 0004; Make 80 student contacts per year during normal activities to strengthen or make new relationships. These are causal contacts at school or community events and activities with the intention of making contact with students but will generally not be to meet a particular student. This would include attending, but not hosting (see 2.1) (when it is not our turn), Bible Lunch Tuesday (B.L.T.). The Contractor will provide a by name list of contacts made in the monthly report (see 0009). CLIN 0005; Make 10 student contacts per year during normal activities to strengthen or make new relationships. These contacts are relational in nature in that the intention is for the Contractor to plan on meeting a particular student and spending at least 30 minutes with that student. The Contractor will provide a by name list of contacts made in the monthly report (see 0009). CLIN 0006; Provide four (4) one-day long recreational activities for fun, fellowship, and sharing for youth. An example of a daylong recreational activity would be a trip to a water or amusement park. Activities should be coordinated with and approved by the Protestant Religious Education Director. Contractor shall obtain written parental consent and medical release for participants in activities off Fort Riley, KS. CLIN 0007; Compile and prepare four (4) newsletters for publication. The contractor shall report ALL contractor manpower required for performance of this contract. See Contractor Manpower Reporting (CMR) instructions in the Supplemental Information Section. If needed, please refer to the CMR users guide at the website provided in the Supplemental Information Section to assess the level of effort that will be required to comply with this requirement. Reporting must occur by Oct 31 for all contracts that were modified or created during the just completed fiscal year (Oct 1 to Sep 30). If contract performance is completed before Sep 30 of a fiscal year, the data may be entered upon contract completion rather than waiting for the end of the fiscal year. NOTE: Contractor will be paid upon submission of report and provision of proof of said submission to be determined by the Government. Your price shall be FOB destination.
