Description
Department of Veterans Affairs VA MidSouth Healthcare Network (VISN 9) Notice of Intent to Sole Source: The Veterans Affairs Medical Center (VAMC) Mt. Home, P.O. Box 4000, Mountain Home, TN 37684-4000 intends to negotiate a sole source contract with FujiFilm Medical Systems USA, Inc. of Stamford, CT. for the service contract of the FujiFilm Digital X-ray machines located at VAMC Mt. Home and the Knoxville Outpatient Clinic in Knoxville, TN. The government believes that FujiFilm Medical Systems USA is the only authorized provider of inspection, service and repair of the two Fuji digital X-ray systems. This is a sole source procurement utilizing, 38 U.S.C. 8153 and Other than Full and Open Competition (FAR Part 6.302-1). This notice of intent is NOT a request for competitive quotes, however; any firm that believes it can provide these services may give written notification to the Contract Specialist, no later than 4:00 PM CST, October 7, 2011. Supporting evidence of ability to provide technically qualified service for both pieces of X-ray equipment must be furnished in sufficient detail, to the Contract Specialist Craig Ziegemeier via email address: craig.ziegemeier@va.gov or faxed to (615)225-5431. No solicitation documents are available and telephone requests will not be honored. Responses received will be evaluated; however a determination by the government to compete the proposed procurement based upon responses to this notice is solely within the discretion of the government. If no responses are received, VA will proceed with the sole source negotiation to FujiFilm Medical Systems USA for the required service. DESCRIPTION/SPECIFICATION/WORK STATEMENT STATEMENT OF WORK PART A - GENERAL INFORMATION A.1. INTRODUCTION - This requirement is for providing Preventative Maintenance Services for FujiFilm Digital X-ray equipment located at the VA Medical Center, Mountain Home, TN and the Knoxville Outpatient Clinic in Knoxville, TN. A.2. BACKGROUND - FujiFilm Digital X-ray equipment at both the VAMC Mt. Home and Knoxville Outpatient Clinic require annual maintenance. A.3. SCOPE OF WORK - "Contractor shall furnish necessary travel, labor, materials, tools, documentation and parts required for the inspection, support and repair of the FujiFilm Equipment, software and accessories listed in the Schedule of Equipment below. "Scheduled and Emergency Service and Travel performed between the hours of 8:30 a.m. and 5:00 p.m. Monday through Friday. "One (1) Preventative Maintenance inspection and calibration per year, unless otherwise specified. "All replacement parts, labor and travel included. Excludes consumable items, batteries, and X-ray tubes. "All software updates based upon original OS & Application purchased configuration. Excludes software/hardware performance upgrades. "One (1) hour telephonic response and four (4) hour on site response for hard down systems. STATEMENT OF WORK PART B - SUPPORTING INFORMATION C.1 Place of Performance - VA Medical Center, Mountain Home, TN and Knoxville Outpatient Clinic, Knoxville, TN C.2. Period of Performance - October 1, 2011 through September 30, 2012. C.3. Special Considerations - N/A C.3.1. Contractor Furnished Materials - All materials and tools to complete the work. C.3.2. Government Furnished Materials and Services - N/A C.3.3. Qualifications of Key Personnel - Factory Trained FujiFilm Field Service Engineers. C3.4. Security Requirements - Work on a government installation may require the need for the contractor's employees to be fingerprinted. VA Medical Center Mt.
