Description
Sole Source Justification This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is F3ZT931227A003; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-53 effective 05 Jul 2011; Defense DPN 20110920 effective 20 Sep 2011, and AFAC 2011-0714 effective 14 Jul 2010. This acquisition is unrestricted; The North American Industry Classification System (NAICS) code is 611430. The business size standard is $7.0M. The Federal Supply Class (FSC) is U099. The Standard Industrial Classification (SIC) is 8299 Travis AFB has a sole source requirement for Consulting and System redesign services. CLIN0001: CHI CONSULTANT AND SYSTEM REDESIGN: A faculty consultant to conduct an on-site System Redesign, coordinate Model Implementation and provide technical assistance for the duration of agreement.(to include consultant travel and expenses) -LEVEL 1 (BASIC) TRAINING FOR UP TO 15 PARTICIPANTS. -LEVEL 2 ADVANCED FACILITATION FOR UP TO 15 GROUP LEADERS. Expenses for the CHI faculty workshop leader included. -Consultation visit with final approval report from Centering Pregnancy. ( to include consultant travel and expenses. -Centering Healthcare Institute annual membership for two years. Includes site listings on CHI website, use of Centering Pregnany name and logos, and free standard shipping on all materials orders. -MATERIALS: 15 Implementation Workbooks,15 Facilitators' Monograps, 15 Facilitators' Guides, Recruitment Kit wiht multi- and single -use materials and DVD,Cenetering Handbook, 50 Centering Mom's Notebooks and canvas bags, Leader kit for activities and group engagement Benchamrking tools to collect and report data Award shall be made in the aggregate, all or none. IAW FAR 6.302-1, all responsible sources may submit a capability statement, proposal or quotation with shall be considered by the agency. The following provisions and/or clauses apply to this acquisition.
