Addendum 1 corrects the Proposal Due Date to read 'October 28, 2011' throughout the RFP package. The primary point of contact for this procurement is Gene Stott (not Joe Burks). REQUEST FOR PROPOSAL 171790 - RADIATION PORTAL MONITORING SYSTEMS The Battelle Memorial Institute, Pacific Northwest Division (Battelle), operator of the Pacific Northwest National Laboratory (PNNL) for the U.S. Department of Energy (DOE), is interested in receiving technical and pricing proposals from your firm to provide Radiation Portal Monitoring (RPM) Systems. This is a competitive solicitation. PNNL intends to award one or more Fixed Price Indefinite Delivery Indefinite Quantity Contracts, for this requirement. All questions related to this Request for Proposal (RFP) must be submitted in writing via email to: gene.stott@pnnl.gov. Formal responses will be provided to all known potential offerors by email in the form of an RFP amendment. This RFP includes the following attachments: Attachment 1: Proposal Preparation Instructions Attachment 2: Intention to Submit a Proposal Form Attachment 3: Sample Contract Schedule Attachment 4: Statement of Work Attachment 5: Performance Specifications Attachment 6: General Provisions and Supplement Attachment 7: Fixed Unit Price Form Attachment 8: Representations and Certifications Form 400 and Supplement Attachment 9: Subcontracting Plan Attachment 10: Quality Assurance Clauses Attachment 11: Prototype Test Plan Overview This Request for Proposal (RFP) consists of two proposal submittals and evaluation phases. • Phase 1: Offerors shall submit qualification documentation related to manufacturing facility capabilities and personnel capabilities as well as past performance information related to the manufacture and delivery of RPM systems. Specific Phase 1 proposal requirements are included in the attached Proposal Preparation Instructions (Attachment 1). The Phase 1 proposal due date is October 28, 2011. • Phase 2: Due to limited space at the Pacific Northwest National Laboratory (PNNL) and Los Alamos National Laboratory (LANL) test facilities and the extensive time and resources required to support the prototype testing, only a limited number of qualified offerors will advance to Phase 2, as determined based on the results of the Phase 1 proposal evaluation. Offerors shall submit technical and price proposals, as well as a prototype RPM system for testing at PNNL and LANL. The Phase 2 proposal due date will be determined following the completion of the Phase 1 proposal evaluation process. The price proposal, the technical proposal and the delivery of the prototype systems for testing will be due 90 days after the release of an amendment to the RFP requesting Phase 2 proposals.
