- NOTE: THIS IS AN AMENDMENT TO THE FOLLOWING COMBINED SYNOPSIS/SOLICITATION. PLEASE OBSERVE THE REVISION TO THE NAICS CODE AND SITE VISIT INFORMATION BELOW. THE COMBINED SYNOPSIS/SOLICITATION IS HEREBY REOPENED. THE RESPONSE DUE DATE IS TUESDAY, 27 SEP 2011, 6:00 P.M. EASTERN STANDARD TIME. THOSE THAT HAVE PROVIDED A QUOTATION IN RESPONSE TO THIS RFQ MAY RESUBMIT A NEW QUOTAION IN ACCORDANCE WITH THE APPLICABLE NAICS. INSTALLATION OF ALL ITEM REQUESTED IS REQUIRED. ***A SITE VISIT IS SCHEDULED FOR MONDAY, 26 SEP 2011 @ 9:00 A.M. ALL THOSE IN ATTENDANCE MUST FIRST MEET AT ANDERSEN AFB VISITOR'S CENTER NLT 8:15 A.M. AS THE ACTUAL SITE ITSELF IS AT ANOTHER LOCATIONS (SHORT DRVING DISTANCE). SHOULD YOU REQUIRE ASSISTANCE OBTAINING A DAY PASS, PLEASE REFER TO THE FOLLOWING INSTRUCTIONS: NOTE: An escort is only permitted to sponsor up to 7 persons at one time. The day of the site visit, you'll need to go to the base Visitor's Center (adjacent to main gate). All requesting a day pass must have a valid ID (driver's license, passport, etc.), permanent resident card (non U.S. Citizen), and proof of insurance and valid registration for designated vehicles entering the base. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, F1C0BW1235A001, is being issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-53. This procurement is a 100% Small Business set-aside IAW FAR 19.502-2 (a). The NAICS code is 423210 and the small business size standard is 100 in accordance with 13 CFR 121.406. The commercial items are requested in this solicitation are identified in the PDF attached, "Furniture Descriptions for Bldg. 25024, 36WG/CVN". Also, refer to the respective building diagrams attached, Bldg. 25024 Office Furniture Layouts". Items should be shipped 30 Days ADC, FOB: Destination for delivery to Andersen AFB, Guam. The following provisions and clauses apply to this acquisition: FAR 52.211-6 Brand Name or Equal. Requested items are brand name or equal. Equal items must include all requested parts and measurements. If submitting an equal brand, offers must include a complete description with catalog cuts to include military specifications. All offers must include all items listed above to be technically acceptable. The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items. Quotes will be evaluated and awarded to the offeror with the offering best value to the government. The provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, and the offeror must include a completed copy of this provision with their proposal. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items. Specifically, the following clauses cited are applicable to this solicitation: FAR 52.204-7 Central Contractor Registration; FAR 52.219-6 Notice of Total Small Business Set Aside; FAR 52.222-26, Equal Opportunity; FAR 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era; FAR 52.222-36, Affirmative Action for Handicapped Workers; FAR 52.222-37, Employment Reports on Disabled Veterans and Veterans of the Vietnam Era.
