Octrode Trial Lead 60cm P/N 3086 4 ea Notice to Contractor: Solicitation number N00259-11-T-0636 is issued as a notice to procure from only one source IAW FAR 13.106-1(b). This proposed contract action is used for used for Anesthesiology/Pain Management Department for 1. EON MIN IPG P/N 3788 2 EA 2. Octrode Perm Lead 60cm P/N 3186 4 ea 3. Eon Min Charger P/N 3721 2 ea 4. Programmer P/N 3851 2 ea 5. Cinch Anchor P/N 1194 10 ea 6. Swift lock Anchor P/N 1192 10 ea 7. Octrode Trial Lead 60cm P/N 3086 4 ea. The Naval Medical Center San Diego intends to solicit with only one source under the authority of FAR 13.106-1(b). The source is St. Jude Medical Inc. CAGE Code: 76905, NAICS: 334510. Justification: As required by FAR 13.106-1(b), the Contracting Officer has determined that St. Jude Medical Inc is the only company that makes these particular consumable items and implants for spinal injuries that the surgeon use for these special types of procedure. IAW FAR 5.207(c) (15) (ii), all responsible sources may submit a quotation, which shall be considered by the agency. The Government will only consider firm, fixed price quotations. The attached solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-53. It is the contractor ™s responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at www.acquisition.gov and/ or www.acq.osd.mil.
