- ATTACHMENT 2 - IUID Tags and Delivery Instruction ATTACHMENT 1 - Band Instrument List, Quantity, Location COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS. A. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The requirement is for DoD Education Activity DDESS School, Maxwell Air Force Base Elementary-Middle School, located in Montgomery, Alabama. This solicitation uses Simplified Acquisition Procedures (SAP) authorized in FAR Subpart 13.5 and is expected to result in the award of one or partial, firm-fixed unit priced contract using commercial contracts standard format 1449. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation will not be issued. B. HA603111RCGS179 is a request for quotation (RFQ): its document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-53. This procurement is 100% set aside for Small Business. NAICS CODE: 451140. Size Standard: $10M. C. PROCUREMENT is for band musical instruments to support the DoD Education Activity (DoDEA) DDESS School, Maxwell AFB Elementary-Middle School, Montgomery AL 36112-6147. D. This is a BRAND NAME or EQUAL acquisition and therefore all vendors who can provide musical instruments, as specified in the ATTACHMENT 1 and deliver in the quantities and to the destination indicated in the solicitation, are invited to submit a quote. If the item has a Brand name, models are specified on each line item. Please state your discounts. E. DESCRIPTION OF REQUIREMENT. a. ATTACHMENT 1 provides specifications, quantities and destination. The attachment is a spreadsheet. Complete the attachment with your best prices. b. Point of Contact for delivery will be provided upon award. Coordination with Point of Contact prior to delivery is required. (See ATTACHMENT 2). c. IUID Tag Labels are required for instruments. Instructions are at ATTACHMENT 2. Further information will be coordinated with Accountability Officer as noted in ATTACHMENT 2. F. SUBMISSION OF OFFERS. 1) RFQ CLOSING DATE. (a). Responses to this RFQ are due no later than 12:00 PM Eastern Standard Time on 26 September 2011. (b) Quotes are submitted by EMAIL to Anna Barbuggiani at anna.barbuggiani@hq.dodea.edu. No other method will be accepted. (c) Attachment 1 and 2 are included to this document. 2) RFQ QUESTIONS concerning this requirement must be submitted by e-mail to Anna Barbuggiani at anna.barbuggiani@hq.dodea.edu no later than 12:00 AM Eastern Standard Time 23 September 2011. Questions will not be responded to after that date. Alternate point of contact, Nella Valdez, nella.valdez@hq.dodea.edu G. SUBMISSION OF QUOTE - ADDITIONAL INSTRUCTION. a. Include the following information in your quote: (1) Cage Code/NAICS Code, DUNS, (2) Email Address for point of contact, Point of Contact Name. b. STATE your best price, providing any and all discounts on products and warranties required for this contract. c. Delivery Schedule. (1) SHIPPING will be FOB Destination. (2) Shipment will be to Maxwell Elementary-Middle School, Montgomery AL 36112. (3) POC of contact at Maxwell will be provided in award. (4) State your best delivery date after receipt of order (ARO). H. AWARD. Government will award a firm-fixed priced order on an all or none basis pursuant to the award criteria stated in EVALUATION (paragraph J(ii) below) of this document. Quotes for less than items solicited will be rejected as non-conforming and unacceptable. I. PAYMENT - ELECTRONIC INVOICE SUBMISSION. DoDEA has implemented Wide Area Work Flow Receipt and Acceptance (WAWF-RA) as its method of electronic submission of invoices and government inspection and acceptance. WAWF instructions are provided in award using 2-in-1 submission. J. CLAUSES. The following clauses/provisions in their latest editions apply to this solicitation: (i) FAR 52.212-1, Instructions to Offerors with the addendum that: "The offer submitted shall include: (1) Proposed prices; (2) completed copy of FAR 52.212-3,"Offeror Representations and Certifications-Commercial Items." NOTE: In lieu of submitting filled-in FAR 52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS-COMMERCIAL ITEMS (AUG 2007), the vendor may fill-in the annual certifications at https://orca.bpn.gov/ and just state in the quote if this was done for the Government to verify; and (3) Cage Code, DUNS and TIN. (ii) FAR 52.212-2, Evaluation, with the following addendum for the basis of award: "The Government will award a contract resulting from this solicitation to the low priced technically acceptable vendor. To be considered technically acceptable, the vendor must be able to fully provide required specifications as stated in Attachment 1." (iii) FAR 52.212-3, Offerors Representations and Certifications; a completed copy of which shall be submitted with the offer. NOTE. See paragraph J(i)(2) above.
