Description
Pittsburgh PA Locations The Social Security Administration intends to make an Unrestricted Acquisition for on-site Adult Influenza Inoculations for various Social Security Administration sites located in the Pittsburgh Pennsylvania area. The successful vendor must supply and administer the Adult Influenza Inoculations to the staff This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations, Sub-part 12.6 and the procedures in FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation shall not be issued. Solicitation Number SSA-RFQ-R03-11-1010b and is issued as a Request for Quote(RFQ). The solicitation document and the incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53. All responsible business sources may submit an offer which shall be considered by the Agency. Requirement Description: The successful vendor shall provide a maximum of 201 Influenza inoculations and 4 each of Pneumonia Shots as well a Registered Nurse to inoculate the employees of the Social Security Administration in the Pittsburgh Area Field Offices. The attached list reflects reflects the addresses and the number of inoculations required for each site. The number of Influenza Inoculations are an estimate only, but shall not exceed the identified 201 inoculations. In addition to the 201 flu inoculations, the vendor shall provide 4 each of Pneumonia Shots- Payment for these shots shall be made by the employee. The vendor shall provide a price for these shots. 1 each of the Flu Inoculations shall also be self -paid The vendor shall: 1. Supply the current Adult Influenza Vaccine and a qualified health-care professional, such as a registered nurse to administer the vaccine to SSA employees. 2. Innoculations will be given at multiple Social Security Administration Sites throughout the Pittsburgh area. Please see attachment for location addresses. 3. The successful vendor shall be paid only for the number of inoculations provided. 4. The vendor shall be responsible for providing a certified statement with a list showing the names and dates of the Social Security Employees received their inoculations. This certified list and statement shall be bumped against the Agency's list. 5. Upon award, the successful vendor shall make contact with the Contracting Officer Technical Representative(COTR) to establish the dates and times of these clinics. The clauses at FAR Part 52.212-4, Contract Terms and Conditions-Commercial Items(JUN2010) applies to this acquisition. The clauses at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items(AUG2011) applies to this acquisition and the additional FAR clauses cited in the the clause applicable to the acquisition: (b)(8),(19),(20),(21)(36)(38). INSTRUCTIONS TO VENDORS- The following provisions at FAR 52. 212-1, Instructions to Offerors (JUN2008) shall apply to this acquisition The Government shall award a firm,fixed price purchase order. All quotations shall clearly marked with the solicitation number SSA-RFQ-R03-11-1010b and shall contain the following: 1. Identification of the Flu Vaccine and the method to be employed to administer same. 2. Price per inoculation. This should include the cost of the vaccine and the cost to administer same. 2a. The price for the Pneumonia shot, which shall be self paid. Vendor may establish method of payment. 3. References for similar work performed with in the last 3 years. The references shall include a current point of contact and telephone number. The Government shall not make more than 2 attempts to contact the references. 4. Certifications and Representations 5. DUNS Number and Tax Identification Number PAYMENT: Payment shall be made by Government Purchase Card for only the number of influenza inoculations provided to the facility- No exceptions shall be made. EVALUATION: To provisions at FAR Part 52.212-2, Evaluation, Commercial Items are applicable to this acquisition. Basis of Award: An award will be made to the vendor offering the lowest priced, technically acceptable quote. To be technically qualified: -The vendor shall be able to provide both the vaccine and a qualified health-care professional, such as a Registered Nurse, to administer the vaccine on site in accordance with the attached address list. The site address(es) are as follows: See Attached List for Site Addresses -Set up on-site clinics at the sites as indicated on the attachment. Provide both the Registered Nurse and Adult Influenza Serum. Dates and times of said clinics shall be established after award. - Offer lowest price REPRESENTATIONS AND CERTIFICATIONS: Quoters shall complete the annual representations and certifications electronically via the ORCA website at https://orca.bpn.gov. If the vendor has already filed his representations and certifications under ORCA, then he must certify that submission of these on-line certifications and representations are accurate and complete and in accordance with FAR Part 52.212-3 have been entered, and uploaded within the last 12 months. All quotes shall be submitted electronically no later than the posted due date and time to the following email address. Vendors shall include all of the required items or the quote shall not be accepted. No paper Request for Quote shall be acknowledged or honored.
