Description
COMBINED SYNOPSIS/SOLICITATION Vibration Shaker System This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number VA-244-11-RQ-0712 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Regulation and Veterans Administration Acquisition Regulation. This solicitation is being issued on an unrestricted basis to allow for full and open competition. The associated NAICS code is 334519. Pursuant to FAR 13.5, this acquisition shall comply with the Test Program for Certain commercial Items. Award will be conducted and evaluated under the provisions of FAR Part 12, Commercial Items, and FAR 13.5, Simplified Acquisition Procedures. The responsible offerors must be registered in the Online Representations and Certifications (ORCA) database (available at: http://orca.bpn.gov/) system per FAR 52.212-3, and submit a completed copy of the Offeror Representations and Certifications - Commercial Items with their quote. Offerors must also be registered in the Central Contractor Registration database (available at: www.ccr.gov) per FAR 52.212-1. Lack of registration in CCR will qualify contractor as ineligible for award. All responsible sources may submit a quotation in response to this solicitation, which shall be considered. This solicitation is being issued as a Request for Quotation (RFQ), for a firm fixed type contract based on the Lowest Price Technically. The VA Pittsburgh Medical Center intends to award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government (best value). This is a Brand Name or Equal purchase description. As such, the provision at FAR 52.211-6 Brand Name or Equal (Aug 1999) applies to this RFQ. This acquisition is for the following items as identified in each Contract Line Item Number (CLIN). STATEMENT OF WORK The VA Medical Centers Human Engineering Research Laboratories is in need of a Vibration Shaker System the will provide HERL researchers with a high-fidelity virtual reality test-bed to simulate real-world community environments for wheelchair users. Upon delivery of purchased items, Contractors will be responsible for assembling and calibrating all components delivered. The contractor will be required to use their own equipment for the installation. They may need to attach components to any or all of our utilities including electrical, pneumatic, and water. Installation will need to occur during regular business hours 8am-4pm Monday thru Friday. The POC for delivery and installation will be Zach Mason 412-822-3666 or zdm7@pitt.edu. A temporary/visitor access pass will be necessary to gain access to the building. That can be obtained by signing in at the front desk. The contractor will be escorted and supervised while performing work. There will be no access given to any VA Networks for installation. This requirement will be evaluated based on Lowest Price Technically Acceptable. To be deemed Technically Acceptable all Offers must be able to prove to be capable of providing quality equipment to include installation by providing literature to support products and services that are being offered.
