Description

This is a combined synopsis / solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION DOCUMENT OFFERED. QUOTATIONS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. This solicitation VA-246-11-RQ-0554 is issued as a request for quote (RFQ). Award will be made using simplified acquisition procedures in accordance with FAR part 13.5. No telephone requests will be accepted. Only written requests received directly from the requester are acceptable. It is the Offeror's responsibility to ensure that the quote is received. The associated North American Industry Classification System (NAICS) code for this procurement is 334516. REQUESTERS MUST STATE THEIR BUSINESS SIZE ON EACH REQUEST. This action is unrestricted. If offeror has a FSS/GSA contract, please include it on your offer. See below for a list of items required. The Department of Veterans Affairs, Salisbury, NC is seeking to enter into a Reagent Rental agreement for HPV (Human papillomavirus) testing for 2012 (Service period 10/1/2011 through 9/30/2012). The medical center currently utilizes Thin Prep* equipment and supplies. The ThinPrep Pap Test is approved by the U.S. Food and Drug Administration (FDA) as a platform for HPV DNA testing directly from the ThinPrep Pap Test collection vial using the QIAGEN Hybrid Capture ® 2* system. The reagent rental agreement includes lease of equipment, Pap supplies and non-Gyn supplies (vials, brushes, cytolyte, preservcyte, slides, and filters). The costs are tied to workload. Projected utilization at the Salisbury VAMC for FY2012 is as follows: Pap Testing: Total Pap Volume/ MonthPrice per Test 83 Non-Gyn Testing: Total Filter Volume/ MonthPrice per Filter 84 *Product names above are Hologic/Cytyc and are being provided for informational purposes. This is a name brand or equal requirement for HPV DNA testing using liquid-based cytology. We are seeking a test methodology that is also approved for Chlamydia trachomatis (CT) and Neisseria Gonorrhea (NG) testing directly from the same collection vial. If another testing method other than the ThinPrep is being proposed, a full product description, including methodology, rate of HSIL detection, error rate shall be submitted in sufficient technical detail to allow the Government to determine the item to be at least equal to the brand name items listed above. Offers without this information cannot be evaluated and the proposal/offer will be determined to be non-responsive. A detailed description of agreement terms and conditions, any maintenance services, and customer service shall be included with the quote. Submitted quotations shall include the RFQ number, the proposed price per test, company name, and point of contact, address, phone number, fax number, tax identification number, DUN number, and business size. Place of performance address is: Salisbury VA Medical Center, 1601 Brenner Avenue, Salisbury, North Carolina. 28144. Attention: You must be registered in CCR to be considered for award per FAR 52.204-7, Central Contractor Registration (April 2008) (b) (2) the offeror shall provide its DUNS, or if applicable, it's DUNS+4 number with its offer, which will be used by the contracting officer to verify that the offeror is registered in the CCR database.
